上消化道不适患者舌象与Hp感染的相关性研究

摘　　要：[目的]探讨上消化道不适患者舌象与幽门螺杆菌（Hp）感染的关系。[方法]采用前瞻性横断面分析的流行病学研究方法,在查阅文献,综合相关专家意见的基础上,设计舌象调查表,对2013年12月~2015年3月因上消化道不适就诊于北京大学第一医院中医、中西医结合科门诊,符合纳入标准的1 112例患者采集舌象信息,建立数据库,运用SAS 8.3软件进行统计学分析。[结果]在1 112例上消化道不适患者中：①Hp检出率与舌色具有相关性,不同舌色Hp检出率由高到低依次为暗红舌〉绛红舌〉鲜红舌〉淡红舌〉紫（瘀斑）舌〉淡白舌〉淡暗舌;②Hp检出率与舌苔具有相关性,不同舌苔Hp检出率由高到低依次为黄腻苔〉薄黄苔〉白腻苔〉少（剥）苔〉薄白苔;③淡白舌＋淡暗舌＋紫（瘀斑）舌与淡红舌比,Hp检出率差异具有统计学意义;④腻苔与非腻苔、白腻苔与黄腻苔、薄白苔与薄黄苔、白苔与黄苔分别比较,Hp检出率差异均具有统计学意义。[结论]Hp感染患者以舌质暗红、绛红、鲜红、淡红,舌苔腻、黄多见。上消化道不适患者舌象与Hp感染具有明显相关性。[Objective]To study the relation between the tongue appearance of patients with upper gastrointestinal symptoms and Hp infection.[Methods]Based on the literature consulting and comprehensive expert advice,the survey questionnaire of tongue appearance was designed according to the prospective and cross-sectional analysis epidemiological investigation methods.From December 2013 to March 2015,1112 cases of hospitalized patients with upper gastrointestinal symptoms from traditional Chinese medicine and Integrated Traditional Chinese Medicine Department of Peking University First Hospital met the inclusion criteria and were acquired tongue manifestation information.The survey database was established and statistically analyzed by software SAS 8.3.[Results]Among the 1 112 cases of hospitalized patients with upper gastrointestinal symptoms：① Hp detection rate and tongue colors were relevant.The frequencies of the tongue colors were as follows：dark-red tongue,deep-red tongue,bright red tongue,light red tongue,purple color or with ecchymoses of the tongue,pale tongue and dull tongue.②Hp detection rate and tongue coating were related.The frequencies of the tongue coating were as follows：yellow greasy fur,thin yellow tongue coating,white greasy fur,less or exfoliative fur,thin and white fur.③Comparied with light red tongue,Hp detection rate among pale tongue,dull tongue and purple color or with ecchymoses of the tongue,obvious statistic differences were observed.④Statistic differences of greasy coating vs.non-greasycoating,white greasy fur vs.yellow greasy fur,thin and white fur vs.thin yellow tongue coating and white tongue coating vs.yellow tongue coating were detected.[Conclusion]Hp infection were mainly seen in patients with dark-red tongue,deep-red tongue,bright red tongue and light red tongue while greasy coating and yellow tongue coating.Hp infection had an obvious correlation to tongue appearance.
