摘 要:作者用热解碳翼形双小叶心瓣(内径16mm)在山羊体内置换的前、后,测量左心房、室压力曲线和升主动脉流量曲线计算出经瓣血流量、经瓣压力降和心瓣有效开放面积,并将其结果与作者已报道的YPV-Ⅰ型牦牛心包瓣膜(内径20mm)测量的结果进行比较,作出其血流动力学的评价。本心瓣置换后的经瓣平均压力降显著为低(P<0.01),心瓣有效开放面积有低的倾向(0.05<P<0.1)。如两种人工心瓣内径相同,双小叶心瓣在血流动力学方面将优于牦牛心包瓣腹。Pyrolytic carbon cambered bileaflet valvedeveloped by Chengdu University of Tech-nology and Sciences was evaluated for itshemodynamics. The curved surface of thevalve leaflet is cylindrical. Under anaes-thesia, cardiopulmonary bypass and chemi-cal cardioplegia, a cambered bileaflet valve(i.d.=16 mm)was replaced at mitral posi-tion in each of 12 goats. Before valve re-placement, mitral flow rate (MFR) was 74.9±12.3ml·sec^(-1), average valvular pressure dropΔP) 0.56±0.17kPa(4.2±1.3mmHg) and effec-tive opening area (EOA) of the valve 1.20±0.18cm^2. After valve replacement, corres-ponding hemodynamic parameters were49.3±12.7ml·sec^(-1), 0.46±0.17kPa (3.8±1.3mmHg) and 0.81±0.12cm^2 respectively. Inour previous report on yak pericardiac heartvalve (i.d.=20mm) in goats, correspondinghemodynamic parameters after valve replace-ment were 45.6±11.8ml·sec^(-1), 0.86±0.32kpa (6.4±2.4mmHg) and 0.65±0.26cm^2. Withcomparable mitral flow rate and heart rate(113.6±19.6min^(-1) vs 119.1±17.1min^(-1)), theaverage valvular pressure drop of pyrolyticcarbon cambered bileaflet valve is signifi-cantly smaller than that of the yak perica-rdiac valve (P<0.001), and the effectiveopening area is marginally larger than thatof the yak pericardiac valve (0.05<P<0.1).If the same-sized valves of this two kindsare used, the hemodynamic parameter ofcambered bileaflet valve would be betterthan those of yak pericardiac valve. Sincethe latter valve has been successfully usedin patients, the present valve might be ap-plicable in clinical situation.
