外固定器联合封闭负压引流技术对胫腓骨骨折患者术后感染的有效性评价

摘　　要：目的观察并探讨外固定器联合封闭负压引流术(VSD)对胫腓骨骨折(OTFF)患者术后感染的控制效果,为临床骨科患者预防术后感染提供参考依据。方法选择2011年2月-2015年1月医院OTFF手术后发生感染患者50例,随机将其分为观察组和对照组,各25例;观察组在外固定器基础联合VSD治疗,对照组给予传统持续灌注冲洗治疗;比较两组患者临床资料及疗效率,数据采用SPSS 19.0软件进行统计分析。结果观察组患者感染控制时间、创面清洁时间和骨折愈合时间分别为(11.8±1.5)d、(23.8±14.2)d和(8.3±1.2)个月,均明显短于对照组的(21.2±1.8)d、(46.5±15.6)d和(14.3±4.8)个月;患者治疗后总有效率观察组为92.0%、对照组为68.0%,两组比较差异均有统计学意义(P<0.05)。结论外固定器联合VSD对OTFF患者术后感染控制效果显著,缩短创面愈合与骨折愈合时间。OBJECTIVE To observe the effect of external fixator combined with vacuum sealing drainage（VSD）on control of postoperative infections in patients with open tibia and fibula fracture（OTFF）patients so as to provide guidance for prevention of postoperative infections in patients of orthopedics department.METHODS A total of 50 OTFF patients with postoperative infections who were treated in the hospital from Feb 2011 to Jan 2015 were enrolled in the study and randomly divided into the observation group and the control group,with 25 cases in each.The observation group was treated with external fixator combined with VSD,and the control group was given the traditional continuous perfusion flushing.The clinical data and the effective rate of treatment were observed and compared between the two groups of patients,and the statistical analysis of data was performed with the use of SPSS 19.0software.RESULTS The time of control of infections of the observation group was（11.8±1.5）days,significantly shorter than（21.2±1.8）days of the control group;the time of wound cleaning of the observation group was（23.8±14.2）days,significantly shorter than（46.5±15.6）days of the control group;the fracture healing time of the observation group was（8.3±1.2）months,significantly shorter than（14.3±4.8）months of the control group.The total effective rate of treatment of the observation group was 92.0%,the control group 68.0%,and there was significant difference between the two group（P〈0.05）.CONCLUSION The external fixator combined with VSD can achieve significant effect on control of the postoperative infections in the OTFF patients and shorten the time of wound healing as well as the fracture healing time
