摘 要:目的探讨慢性根尖周炎感染根管中微小小单胞菌与牙髓卟啉单胞菌定植的关系,为后期研究提供参考依据。方法选取2014年1-12月医院就诊的80例慢性根尖周炎患者为研究对象,采集所有患者100颗患牙根管内标本,其中包括35例首次治疗患者,有50颗患牙,作为首次治疗组,45例根管再治疗患者,有50颗患牙,作为根管再治疗组;计算标本中微小小单胞菌DNA检出率,并分析微小小细胞菌与牙髓卟啉单胞菌、粪肠球菌和牙龈卟啉单胞菌之间的相关性。结果首次治疗组患者的微小小单胞菌、粪肠球菌、牙髓卟啉单胞菌、牙龈卟啉单胞菌检出率分别为57.1%、0、45.7%、22.9%,根管再治疗组的检出率分别为6.7%、13.3%、8.9%、2.2%,差异有统计学意义(P<0.05);首次治疗组和根管再治疗组患者标本中微小小单胞菌与牙髓卟啉单胞菌的检出率相关,但与粪肠球菌、牙龈卟啉单胞菌的检出率无相关性。结论微小小单胞菌与牙髓卟啉单胞菌同为慢性根尖周炎感染根管微生态环境中的主要致病菌,且二者在感染根管内的定植可能为共生关系。OBJECTIVE To explore the relationship between colonization of Parvimonas micra and Porphyromonas endodontalis in root canals of patients with chronic apical periodontitis so as to provide guidance for the subsequent study.METHODS A total of 80 patients with chronic apical periodontitis who were treated in hospitals from Jan2014 to Dec 2014 were recruited as the study objects,then the specimens inside the root annals were collected from100 teeth,50teeth from 35 patients who were treated for the first time were assigned as the primary treatment group,and 50 teeth from 45 patients who suffered from secondary root canals infections were set as the root canal retreatment group.The detection rate of DNA in the Parvimonas micra was calculated.The relationship between the Parvimonas micra and the Porphyromonas endodontalis,Enterococcus faecalis,Porphyromonas gingivalis was explored.RESULTS The detection rates of Parvimonas micra,E.faecalis,Porphyromonas endodontalis,and Porphyromonas gingivalis were respectively 57.1%,0,45.7%,and 22.9%in the primary treatment group;the detection rates were respectively 6.7%,13.3%,8.9%,and 2.2%in the root canal retreatment group,and there was significant difference(P〈0.05).The detection rate of the Parvimonas micra in the specimens of the patients in the primary treatment group and the root canals retreatment group was associated with the detection rate of the Porphyromonas endodontalis,however,it was not associated with the detection rate of E.faecalis or Porphyromonas gingivalis.CONCLUSIONParvimonas micra and Porphyromonas endodontalis are dominant among the pathogens in micro-ecological environment of root canals of the patients with chronic apical periodontitis,and the colonization of the two species may show the symbiotic relationship in the infected root canals.
