摘 要:目的观察坐骨神经分支选择性损伤(SNI)模型大鼠脊髓背角γ-氨基丁酸(GABA)能神经元比例的变化。方法健康雄性SD大鼠36只,随机分为SNI组(S组)和假手术组(D组),各18只。S组暴露左侧坐骨神经主干及三个分支,结扎切断胫神经和腓总神经,保留腓肠神经。D组仅暴露左侧坐骨神经主干及其分支,不行结扎。术前1天和术后7、14、28天分别进行大鼠左后爪内外侧机械痛阈测试。造模后7、14、28天两组大鼠分批取材(n=6)行4%多聚甲醛心脏灌注,取腰脊髓段(L5)制备石蜡切片,免疫组化染色定位GABA能神经元,并运用体视学形态定量分析两组大鼠脊髓背角GABA能神经元比例。结果 S组大鼠术后7、14、28天左侧脊髓背角GABA能阳性神经元比例分别为60.41%±8.49%、55.27%±6.80%、61.87%±8.12%,右侧脊髓背角GABA能阳性神经元比例分别为61.94%±11.04%、64.68%±9.77%、62.58%±4.25%,D组大鼠术后7、14、28天左侧脊髓背角GABA能阳性神经元比例分别为61.27%±8.73%、63.59%±5.97%、62.81%±10.26%,右侧脊髓背角GABA能阳性神经元比例分别为58.02%±4.58%、60.84%±8.28%、63.74%±10.88%,S组术后14天左侧GABA能神经元比例较同组右侧、D组左侧低,P均<0.05。结论 SNI模型大鼠神经损伤侧脊髓背角GABA能神经元比例降低,但这种改变并不持久,GABA能神经元的转化可能与SNI大鼠机械性痛敏有关。Objective To observe the proportion changes of γ-aminobutyric acid( GABA) ergic neurons in spinal dorsal horn( SDH) of SNI( spared nerve injury) model rats. Methods Thirty-six healthy male SD rats were randomly divided into two groups: SNI group( group S,n = 18) and sham operation group( group D,n = 18). The left sciatic nerve trunk and three branches were exposed,two of which,known as tibial and peroneal nerve,were ligated and cut off,and the sural nerve was preserved to build the SNI model in the group S. The left sciatic nerve trunk and three branches were only exposed in the group D. On the 1st day before surgery,7th,14 th and 28 th days after surgery,the outside and inside MWTs( mechanical withdrawal threshold) of left hind paw in rats were tested. On the 7th,14 th and 28 th days after surgery,rats in the two groups underwent cardiac perfusion of 4% paraformaldehyde,then the L5 spinal segments were dissected to prepare paraffin sections. GABAergic neurons were labeled by immunohistochemical staining of GABA neurotransmitters in cytoplasm. The quantitative analysis of stereological morphometry was used to describe the proportions of GABAergic neurons in the two groups. Results The proportions of GABAergic neurons in the left spinal dorsal horn of group S on the7 th,14th and 28 th days after surgery were 60. 41% ± 8. 49%,55. 27% ± 6. 80% and 61. 87% ± 8. 12%,respectively,and in the right spinal dorsal horn of group S were 61. 94% ± 11. 04%,64. 68% ± 9. 77% and 62. 58% ± 4. 25%. On the same way,the proportions of GABAergic neurons in the left spinal dorsal horn of group D on the 7th,14 th and 28 th days after surgery were 61. 27% ± 8. 73%,63. 59% ± 5. 97% and 62. 81% ± 10. 26%,respectively,and in the right spinal dorsal horn of group D were 58. 02% ± 4. 58%,60. 84% ± 8. 28% and 63. 74% ± 10. 88%. Compared with the right SDH of the same group and left SDH of group D,there were statistically significant reductions in the proportions of GABAergic neurons of the left SDH on 14
