摘 要:目的研究甲基强的松龙对呼吸道合胞病毒(respiratory syncytial virus,RSV)毛细支气管炎患儿外周血Th17/Treg平衡及其细胞因子的影响,进一步探讨甲基强的松龙治疗毛细支气管炎的免疫机制。方法采用流式细胞术测定治疗前后患儿外周血中Th17及Treg细胞的百分率;采用ELISA测定治疗前后患儿外周血中细胞因子IL-17及TGF-β的含量。结果甲基强的松龙治疗前,实验组和肺炎对照组患儿外周血中Th17细胞百分率及IL-17均显著高于正常对照组(P<0.05);而Treg细胞百分率及TGF-β均显著低于正常对照组(P<0.05);实验组与肺炎对照组患儿Th17、Treg细胞百分率及其相关细胞因子之间的差异均无统计学意义(P>0.05)。治疗7天后,实验组和肺炎对照组患儿外周血Th17细胞百分率及IL-17较前均有减少,而Treg细胞百分率及TGF-β较前均有增加;实验组治疗前后Th17、Treg细胞百分率及其相关细胞因子的差异均有统计学意义(P<0.05),而肺炎对照组患儿治疗前后Th17、Treg细胞百分率及其相关细胞因子的差异均无统计学意义(P>0.05)。结论甲基强的松龙可以抑制RSV毛细支气管炎患儿血清中Th17细胞的表达,同时促进Treg细胞的表达,使疾病状态下失衡的Th17/Treg趋于平衡状态,从而减轻RSV感染的毛细支气管炎患儿肺部的免疫损伤程度,快速有效地缓解患儿的喘憋症状。Objective To investigate the influence of methylprednisolone on peripheral blood Th17/Treg balance and cytokines of children with bronchiolitis infected with respiratory syncytial virus(RSV),and to further discuss the immunologic mechanism of methylprednisolone on bronchiolitis.Methods The flow cytometry analysis was used to determinate the Th17 and Treg cells rate in peripheral blood of children with bronchiolitis before and after treatment.The expression of IL-17 and TGF-βbefore and after therapy was detected by ELISA.Results Before the treatment of methylprednisolone,the rates of Th17 cells and IL-17 in peripheral blood of the experimental group and the pneumonia control group were obviously superior to the normal group(P〈0.05),while the rate of Treg cells and TGF-βwas distinctly decreased compared with the normal group(P〈0.05).And the rate of Th17、Treg cells and their related cytokines between the experimental group and the pneumonia control group had no statistic significance(P〉0.05).After having been treated for 7days,the rate of Th17 cells and IL-17 in peripheral blood of the experimental group and the pneumonia control group were reduced,while the rate of Treg cells and TGF-βwas increased.The difference of the rate of Th17 and Treg cells and their related cytokines in the experimental group before and after therapy had statistic significance(P〈0.05),but in the pneumonia control group,it had no statistic significance(P〉0.05).Conclusion Methylprednisolone can suppress the rate of Th17 cells in peripheral blood of children with RSV bronchiolitis as well as promote the rate of Treg cells to adjust the unbalance of Th17/Treg in the morbid state.And as a result,it can relieve asthmatic symptom of patients effectively.
