摘  要:【目的】探讨维持性血液透析(Hemodialysis,HD)患者锌α2糖蛋白(zinc-α2-glycoprotein,ZAG)的表达及其意义。【方法】选择维持性HD患者50例(观察组),同时选择门诊体检者健康对照者30例(对照组)。采用ELISA法检测ZAG、瘦素(LP)和脂联素(ADPN)水平,并检测尿素氮(BUN)、肌酐(Cr)、C反应蛋白(CRP)、空腹血糖(FBG)、空腹胰岛素(Fins)、总胆固醇(TC)、游离脂肪酸(FFA)、高密度脂蛋白(HDL)和低密度脂蛋白(LDL),肾小球滤过率(GFR)、胰岛素抵抗指数(HOMR-IR)。【结果】两组Cr、BUN、GFR比较有统计学差异(P〈0.01),FBG、Fins、HOMR—IR比较无统计学差异(P〉0.05);两组TG、LDL、FFA、AD—PN、LP比较有统计学差异(P〈0.01),TC、HDL无统计学差异(P〉0.05);ZAG与LP、ADPN、HOMR-IR呈正相关(P〈0.05),与CRP、GFR、体质量指数(BMI)、HDL呈负相关(P〈0.05),多元线性回归分析ZAG与HDL、ADPN显著相关性(P〈0.05)。【结论】正常人群中ZAG水平与脂代谢和糖代谢有关,HD患者的ZAG水平高于正常组,锌α2糖蛋白在HD患者血清中的浓度有所增加,通过LP、ADPN等因子,影响患者体质量和脂质代谢,使得体质量减小,脂质堆积减少。[Objective]To explore how maintenance hemodialysis (HD) of patients with zinc (zinc- alpha 2-glycoprotein alpha 2 glycoprotein, ZAG) affects the level change of metabolism in patients with HD. [Methods]Fifty cases undergoing the blood purification treatment of maintenance hemodialysis patients at our hospital were chosen as subjects of study, while clinic examination established 30 healthy cases to serve as the control group. Application of the ELISA detection method was used to detect ZAG, leptin and adiponectin levels, and detection of clinical and biochemical data was used to detect blood Urea nitrogen (BUN), creatinine (Cr), fasting blood glucose (FBG), fasting insulin (Fins), total cholesterol (TC), free fatty acid (FFA), high density lipoprotein (HDL) and low density lipoprotein (LDL), and insulin resistance index. Patient sex, age, dialysis duration, and body mass index (BMI) data were also collected. [Results] Serum creatinine, BUN, and GFR difference was statistically significant ( P 〈 0.01), but there was no significant difference in FBG, Fins, and HOMR-IR between the groups ( P 〉0.05). TG, LDL, FFA, adiponectin, and leptin between the two groups were statistically different ( P 〈0.05), but the difference in TC and HDL was not ( P 〉0.05). ZAG was positively correlated to LP, ADPN, HOMR-IR and inversely correlated to CRP, GFR, BMI and HDL. Furthermore, multiple regression analysis showed that ZAG was very closely related with HDL and ADPN.[Conclusion]In maintenance hemodialysis patients, ZAG levels are higher than that in the normal grotip. ZAG affects lipid and glucose metabolism through LP and ADPN, which in turn reduces BMI and lipid accumulation.
