摘 要:【目的】探讨自制组合套件在经皮椎体成形术(PVP)中的临床应用效果。【方法】选取2015年1~7月在本院接受PVP术的60例患者,随机分为观察组和对照组。观察组(n=32)应用自制组合套件进行穿刺定位完成PVP手术;对照组(n=28)采用传统直接穿刺定位完成PVP手术。对两组的穿刺成功率及效果进行临床评价。【结果】所有患者均完成PVP手术。观察组均为一次性穿刺成功,准确率达到100%,平均透视次数为4次,平均手术时间(6.8±1.2)min。对照组一次性穿刺成功率约67.8%,其中有9例需二次或多次更换穿刺点,平均透视次数9次,平均手术时间(9.6±1.5)min。观察组和对照组术后1周VAS评分分别为(2.5±1.5)分和(2.8±1.6)分,较术前的VAS评分(7.5±1.6)分和(7.9±1.0)分均明显改善,但两组之间比较,差异无统计学意义(P〉0.05)。所有患者均未出现骨水泥向椎管周围渗漏。【结论】应用自制组合套件在PVP术中进行穿刺定位,简单易行,安全准确,可以减少医患接受X线透视次数,缩短手术时间,临床疗效满意,值得临床初学者及基层医院推广应用。[Objective]To explore the clinical application effect of self-made composite suite in the percuta- neous vertebroplasty(PVP). [Methods]Sixty patients who have accepted the PVP from January 2015 to July 2015 in our hospital were enrolled in this study and randomly divided into observation group and control group. Self-made composite suite was applied for puncture and positioning for the observation group (n = 32) to complete the PVP operation; traditional direct puncture positioning was used for the control group (n=28) to conduct PVP surgery. The puncture success rate and clinical effect of the two groups were evaluated.[Results] All patients underwent PVP, 32 patients of the observation group were succeeded in puncture , and accuracy reached 100 %, the average frequency of X-ray 4 times, the average operation time (6.8 ± 1.2) min, among which 9 cases needed to replace the puncture points for twice or more times, the average frequency of X-ray 9 times, the average operation time (9.6±1.5) min. At one week after surgery , VAS score of the two groups were (2.5 ±1.5) and (2.8 ± 1.6 ) respectively, compared with those pre-surgery ( 7.5 ± 1.6 ) and ( 7.9 ± 1.0), there were significant improvement, but the difference between the two had no statistical significance( P 〈0.05). All patients presented no leakage of bone cement around the spinal canal. [Conclusion]Application of this self-made composite suite in PVP operation for puncture positioning is simple, safe and accurate, and is able to reduce the X-ray frequency and shorten the operation time. The clinical results are satisfactory and are worthy introducing to clinical beginners and basic-level hospitals.
