摘　　要：【目的】探讨后腹腔镜肾部分切除术（RLPN）在肾脏小包块（SRMs）中的应用效果和安全性。【方法】回顾性分析2012年7月至2013年12月体检发现SRMs并在本院接受RLPN术的27例患者的临床资料，探讨该术式的疗效及安全性。【结果】所有患者均顺利完成手术，手术时间平均（76．3±12．4）min，肾蒂平均阻断时间（肾脏热缺血平均时间）（23．4±2．8）min，术中出血量（56．7±12．4）mL。术后病理结果提示肾透明细胞癌24例，肾错构瘤3例。透明细胞癌患者肿瘤切缘均为阴性，肿瘤分期为T1a。27例患者随访6～24（17．6±2．3）个月，所有患者均无瘤存活。【结论】SRMs的病理结果多为恶性，需要积极处理。RLPN术能有效保存SRMs患者肾单位，疗效确切，是一种安全、有效、可行的手术方式。[Objective] To discuss the clinical application of retroperitoneal laparoscopic partial nephrectomy （RLPN）for small renal masses. [Method] In this retrospective study, the data of 27 patients with small renal masses were analyzed.[Result] All procedures were technically successful. The mean operating time was （76.3 ±12.4） minutes, mean warm ischemia time was （23.4 ±2.8） minutes, and mean estimated blood loss was （56.7±12.4） mL. No intraoperative or postoperative serious complications were observed. Of the 27 patients, 24 were finally diagnosed with renal cell carcinoma, and had negative surgical margins. Neither local recurrence nor metastasis was observed during follow-up of 6-24 months （average 17.6±2.3）. [Conclusion] Small renal masses seem like malignant, and should not be ignored. For selected patients, retroperitoneal laparoseopic partial nephrectomy can be performed safely and effectively with satisfactory outcomes.
