地市级医院开展体外受精-胚胎移植技术的可行性及有效性研究

摘　　要：目的 探讨地市级医院体独立开展体外受精-胚胎移植技术（IVF-ET）的可行性、有效性及安全性。方法回顾性分析2014年3月-2015年6月我院接受IVF-ET助孕的不孕症患者106例,其中盆腔输卵管因素不孕52例,排卵障碍20例,子宫内膜异位症4例,卵巢储备功能减退9例,不明原因不孕3例,男性不育18例,女性年龄≥35岁27例。根据患者的窦卵泡数、体重指数及年龄等因素采用不同的控制性超排卵（COH）方案。结果常规IVF-ET组89周期,卵胞浆内单精子注射（ICSI-ET）组17周期,鲜胚移植85周期,冻融胚胎移植（FET）21周期。共有64例患者确认妊娠,妊娠率60.4%,临床妊娠流产8例,流产率12.5%,宫外孕2例,宫外孕率3.1%,多胎妊娠20例,多胎妊娠率31.3%,卵巢过度刺激综合征（OHSS）2例,OHSS发生率1.9%。目前已分娩38个婴儿,男婴21个,女婴17个,男女比例1.2：1。结论 IVF-ET技术在地市级医院独立开展是安全可行有效的。Objective To discuss feasibility,validity and security of municipal hospital in independent developing of Vitro Fertilization-Embryo Transfer（IVF-ET） technique.Methods Retrospectively analyzed the 106 cases of infertility patients who receiving IVF-ET help pregnancy from March 2014 to June 2015 in our hospital,including pelvic tubal factor infertility 52 cases,ovulation obstacle 20 cases,endometriosis 4 cases,decreased ovarian reserve function of9 cases and unknown reason infertility in 3 patients,18 cases of male infertility.women age over 35 years old 27 cases.According to the patients＇ different factors such as antral follicle count,body mass index and age,using different controlled ovarian hyperstimulation（COH）.Results Conventional IVF-ET group 89 cycles,intracytoplasmic sperm injection（ICSI-ET） group of 17 cycles,fresh embryo transplantation 85 cycles,frozen-thawed embryo transfer（FET） 21 cycles.A total of 64 patients were confirmed pregnancy,the pregnancy rate was 60.4%;8 cases of clinical pregnancy abortion,abortion rate was 12.5%,2 cases of ectopic pregnancy,ectopic pregnancy rate was 3.1%,20 cases of multiple pregnancy,multiple pregnancy rate was 31.3%,2 cases of ovarian hyperstimulation syndrome（OHSS）,OHSS incidence was 1.9%.Now our hospital delivered 38 babies,including 21 baby boys,17 girls,the sex ratio was 1.2：1.Conclusion Independent developing IVF-ET technology in municipal hospital was safe,feasible and effective.
