湛江市237例慢性阻塞性肺疾病患者抑郁状况及影响因素分析

摘　　要：目的了解慢性阻塞性肺疾病患者抑郁状况及其相关的影响因素,为提高慢性阻塞性肺疾病患者的生存质量和找到抑郁并发症的改善方法,提供科学的依据。方法应用抑郁自评量表（SDS）对湛江市3所医院237例慢性阻塞性肺疾病患者进行问卷调查,把得出的数据录入整理并且进行统计分析。结果 237例慢性阻塞性肺疾病患者无抑郁82例（34.60%）,轻度抑郁98例（41.35%）,中度抑郁57例（24.05%）,无重度抑郁,抑郁总发生率为65.40%。Logistic回归（逐步法）分析显示抑郁症状的发生与年龄[OR=2.54,95%CI：1.45～4.45]和自评健康状况[OR=1.57,95%CI：1.15～2.15]有关。结论慢性阻塞性肺疾病患者的抑郁发生率较高,受到年龄、自评健康状况等因素的影响,应引起社会和家庭的关注,加强心理健康指导,正确引导病人的情绪管理。Objective To understand the depression status and related factors among patients with chronic obstructive pulmonary disease,so as to provide scientific basis for improving their survival quality and finding the way for improving depressive complications. Methods The self rating depression scale（SDS）was used to conduct a questionnaire survey among 237 patients with chronic obstructive pulmonary disease in 3hospitals of Zhanjiang city for data entry and statistical analysis. Results In 237 patients with chronic obstructive pulmonary disease,there were 82 patients without depression（34.60%）,98 patients with mild depression（41.35%）,57 patients with moderate depression（24.05%）,and no severe depression,the total depression rate was 65.40%.Logistic regression analysis showed that the occurrence of depressive symptoms was related to age and self-rating health status,ORand 95% CI were 2.54（1.45-4.45）with 1.57（1.15-2.15）respectively. Conclusion The depression occurrence rate is high among patients with chronic obstructive pulmonary disease and affected by age,self-rating health status and other factors.Society and family should pay more attentions to the patients,strengthen mental health guidance,and take care of the patient＇s mood.
