摘 要:目的观察骨化三醇对骨质疏松家兔血清和骨组织维生素D水平的影响,探讨骨化三醇在骨质疏松症治疗中的作用机制及临床意义。方法采用8月龄雌性新西兰白兔,通过摘除双侧卵巢建立骨质疏松模型,随机分为高剂量组、低剂量组和空白对照组,每组8只,共24只。高剂量组、低剂量组分别给予骨化三醇[1,25(OH)2D_3]0.1μg/(kg·d)和0.025μg/(kg·d)灌胃;对照组给予脂肪乳剂2 ml/(kg·d)灌胃,给予标准饲料喂养。给药12周后测定各组的血清及骨髓中的25-羟基维生素D_3[25-hydroxyvitamin D_3,25(OH)D_3]及1,25-二羟基维生素D_3[1,25-dihydroxy vitamin D_3,1,25(OH)2D_3]水平的变化。结果高剂量组、低剂量组和空白对照组的血清和骨髓25(OH)D_3水平组间差异有显著统计学意义(F=15.075,P<0.001)(F=98.184,P<0.001)。可以看到高剂量组、低剂量组血清25(OH)D_3水平明显高于空白对照组,但是骨髓25(OH)D_3水平明显低于空白对照组。三组的血清和骨髓1,25(OH)2D_3水平组间差异也有显著统计学意义(F=1918.943,P<0.001)(F=2236.508,P<0.001),均是高剂量组、低剂量组高于空白对照组(P<0.001)。血清25(OH)D_3水平总体高于骨髓,差异有统计学意义(F=34.84,P=0.00),血清和骨髓1,25(OH)2D_3水平没有统计学差异(F=-0.687,P=0.495)。结论骨化三醇能够明显提高去卵巢骨质疏松模型家兔骨组织中1,25(OH)2D_3水平,直接作用于骨组织中的成骨细胞和破骨细胞,治疗骨质疏松症。Objective This study through the treatment of different concentrations of calcitriol on primary osteoporosis animal model,to observe the changes of serum and bone tissue 25 hydroxy vitamin D_3 and 1,25- dihydroxy vitamin D_3. Methods Female New Zealand white rabbits aged 8 months were ovariectomized bilaterally as models of postmenopausal osteoporosis and they were randomly divided into 3 groups: high dose group,low- dose group and blank control group. High dose group,low dose group were respectively treated with calcitriol 0. 1 μg /( kg·d)and 0. 025 μg /( kg·d) gavage; the control group was given intragastric administration of fat emulsion 2 ml /( kg·d). After 12 weeks administration,the levels of 1,25( OH)2D_3and 25( OH) D_3 in serum and bone marrow of each group were measured. Results 25( OH) D_3 levels of high dose group,low dose group and blank control group were statistically different( F = 15. 075,P〈0. 001)( F = 98. 184,P〈0. 001),and 1,25( OH)2D_3significant difference also( F = 1918. 943,P〈0. 001)( F = 2236. 508,P〈0. 001). The serum 25( OH) D_3 level was significantly higher than bone marrow 25( OH) D_3level( F = 34. 84,P = 0. 00). Serum 25( OH) D_3 levels of high dose group and low dose group were significantly higher than control group; so as to 1,25( OH)2D_3. Bone marrow 25( OH) D_3 levels of high dose group and low dose group were significant low to control group. Conclusion Calcitriol can significantly improve serum and bone tissues 1,25( OH)2D_3level and directly act on osteoblasts and osteoclasts of bone tissue in ovariectomized rabbit model of osteoporosis.
