单独二孩政策实施前后经产妇妊娠并发症和分娩结局的回顾性分析

摘　　要：目的了解"单独二孩"政策实施后,医院接诊经产妇数量、分娩方式、妊娠并发症和分娩结局的改变。方法对2014年一季度和2015年一季度上海三级专科妇产科医院经产妇的年龄、妊娠间隔、初产分娩方式、妊娠并发症和分娩结局等资料进行回顾性分析。结果 2014年一季度总分娩量3 927例,其中经产妇511例;2015年一季度总分娩量3 886例,其中经产妇823例;经产妇数量同比增加了61%。与2014年一季度相比,2015年一季度高龄产妇、低学历产妇、具有多次流产史的经产妇增多,前次剖宫产史经产妇达到48.85%,妊娠期糖尿病及子痫前期和产后出血大于1 000 ml病例增多;新生儿窒息、早产、低出生体重、巨大儿、入新生儿重症监护室比率方面无明显改变。结论"单独二孩"政策实施后,对临床提出了挑战,需要更好的妊娠期及分娩期管理,以使母儿均得到良好的结局。Objective To explore the influence before and after the change of policy on family planning on number,mode of delivery,pregnancy complications and outcomes in multiparas. Methods The age,interval of pregnancy,mode of delivery,pregnancy complications and outcomes in multiparas had delivery during the first quarter of 2014 to 2015 in an AAA grade special obstetrics and gynecology hospital were retrospectively analyzed. Results During the first quarter of 2014,there were 511 women with birth for their second or third child reached 3 927 at all. At the first quarter of 2015,the number of birth was 823 in 3 886 which increased 61%. In comparison with the multipara who had delivery in the first quarter of 2014,there were more women with advanced age,low education level,having multiple miscarriges in the same time at 2015.The percentage of women with previous caesarean was 48. 45%. The percentage of women with gestational diabetes,preeclampsia and postpartum hemorrhage（ 〉1 000 ml） was higher in 2015. There was no statistical significance on pregnancy outcome including neonatal asphyxia,preterm birth,macrosomia,low birth weight,admission to special care units for infants. Conclusion After the change of policy on family planning,we should face to a greater challenge,hence it needs better method for management during gestation period and delivery.
