神经根型颈椎病疗效与肌电图相关性分析

摘　　要：目的:统计神经根型颈椎病治疗后的疗效,分析神经根型颈椎病治疗后肌电图数据的改变情况,探讨疗效与肌电图数据改善情况的相关性。方法:2012年10月—2015年12月,收集入院肌电图检查异常的神经根型颈椎病64例,并随机分为2组,对照组(推拿+牵引+中药热敷+电脑中频+药物治疗)20例,治疗组(对照组治疗方法+骶管冲击治疗)44例。出院时统计患者疗效,并且复查肌电图,分析疗效与肌电图改善情况的相关性。结果:对照组20例(优5例,良4例,有效8例,差3例)中,疗效与肌电图改变的相关性r=0.121,t=0.516,P>0.05。治疗组44例(优10例,良25例,有效8例,差1例)中,疗效与肌电图改变的相关性r=0.412,t=2.932,P<0.01。两组的疗效有统计学差别(χ2=7.64,P<0.01)。结论:治疗组具有更好的临床疗效,且与肌电图的改善数据呈正相关性;骶管冲击治疗可更快地改善患者的病情或促进神经的恢复。Objective：To count the efficiency of cervical spondylotic radiculopathy and to analyse its electromyogram（EMG） data changes after treatment,then to discuss the correlation between the clinical effects and the improvement of EMG data. Methods：From 2012 Oct to 2015 Dec,64 patients of cervical spondylotic radiculopathy with abnormal EMG were collected and divided randomly into two groups：control group（treated by massage,traction,Chinese medicine fomentation,computer IF and medications）20 cases;therapy group（treated by sacral injection and the rest therapy following the control group）44 cases. All the patients＇ curative effect was compared and theirs EMG was reexamined on discharge,then to analyze the correlation between the efficacy and the improvement of EMG data. Results：In the control group involved 20 cases（5 patients healed excellent,4 patients improved partially,8 patients turned effective,3 patient had poor outcome）,the correlation between the clinical effects and EMG changes showed r=0.121,t=0.516,P〈0.05. In the therapy group involved 44 cases（10 patients healed excellent,25 patients improved partially,8 patients turned effective,1 patient had poor outcome）,the correlation between the clinical effects and EMG changes showed r=0.412,t=2.932,P〈0.01. There was significant distinction between two groups inthe clinical effects（χ2=7.64,P〈0.01）.Conclusion：There was a better clinical effect which has positive correlation with the improvement of EMG data in therapy group. Sacral injection can improve the general condition of patients or promotethe recovery of nerve more quickly.
