臂踝脉搏波传导速度与糖尿病合并冠心病的中医临床相关性研究

摘　　要：目的:通过臂踝脉搏波传导速度(ba PWV)观察糖尿病合并冠心病患者与单纯糖尿病患者中医辨证分型及病机转化相关性。方法:将研究对象分为糖尿病组及糖心病组(糖尿病合并冠心病)两组,根据《中医内科学》制定中医证候观察表,测定所有采集证型患者的ba PWV、性别、年龄、病程,进行ba PWV数值分段等比较,观察是否存在相关性。结果:糖心病组病程、年龄、ba PWV数值、血瘀证、湿热证、气郁证、痰湿证、心虚证例数明显高于单纯糖尿病组(P<0.01),而单纯糖尿病组中气虚、燥热、阴虚例数明显高于糖心病组(P<0.01),糖尿病与糖心病患者ba PWV在1800 cm/s以上更具有差异(P<0.01),ba PWV数值与血瘀证、痰湿证、心虚证显著正相关(相关系数分别为0.328、0.291、0.045)。结论:ba PWV在1800 cm/s以上糖尿病患者更易患冠心病,ba PWV可间接反映糖尿病与冠心病病机转化,对进一步指导临床辨证论治具有深远意义。Objective：To observe the coronary heart disease combined with diabetes（DM＋CHD）and diabetes（DM）patients without heart diseases of traditional Chinese medicine and the transformation of pathogenesis ralationship by using the brachial-ankle pulse wave velocity（ba PWV）. Methods：Research object can be divided into two groups：simple diabetes group and diabetic cardiopathy group（DM＋CHD group）,according to the internal medicine of traditional Chinese medicine,set the TCM syndromes watch list,determination of ba PWV all eligible patients,gender,age,course of the disease,the corresponding TCM syndrome types,compare ba PWV value segmentation etc,see if there were some correlation. Results：The course of the disease,age,ba PW,blood stasis,damp-heat,depressed in Qi,phlegm-damp,heart deficiency case number of coronary heart disease combined with diabetes obviously higher than simple diabetes patients（P〈0.01）,whereas in simple diabetes group deficiency of Qi,dryness-heat,yin deficiency case number higher than diabetic cardiopathy group（P〈0.01）. Diabetic patients with coronary heart disease（CHD）pulse wave conduction velocity section count showed that diabetes and diabetic patients with coronary heart disease ba PWV in more than 1800 cm/s has more differences（P〈0.01）. Ba PWV value and blood stasis syndrome,phlegm dampness syndromes,heart deficiency syndrome were significantly positively related,the correlation coefficient is 0.328,0.291,0.239. Conclusion：Ba PWV in more than 1800 cm/s diabetics are more likely to suffer from coronary heart disease. Ba PWV can indirectly reflect the transformation of diabetes and coronary heart disease pathogenesis,has profound significance for further to guide the clinical diagnosis and treatment.
