胎儿永存左上腔静脉畸形的超声诊断分析

摘  要:目的探讨超声在胎儿永存左上腔静脉畸形中的诊断意义。方法 120例永存左上腔静脉畸形胎儿作为研究对象,使用Wolusion E8彩色多普勒超声诊断仪对胎儿心内、心外结构进行常规检查和测量。观察胎儿肺动脉左侧有无血管及其血流方向和频谱,同时观察其是否合并其他畸形。结果 120例胎儿中119例为双侧上腔静脉,1例为右侧上腔静脉中断。118例左上腔静脉回流到右心房,2例回流到左心房。102例是单纯永存上腔静脉,占85.00%;8例合并室间隔缺损,占6.67%;2例合并房间隔缺损,占1.67%;8例合并左室发育不良综合征,占6.67%。结论根据其特征性表现,早期对永存上腔静脉畸形进行诊断意义重大,可以防止其他先天性疾病的发生。Objective To investigate diagnostic significance by ultrasound for fetal persistent left superior vena cava malformation. Methods There were 120 fetus with persistent left superior vena cava malformation as study subjects. Wolusion E8 color Doppler ultrasound diagnostic system was applied in conventional examination and measurement of intracardiac and extracardiac structure. Observation was made on blood vessel on the left of pulmonary artery, and its flow direction and frequency spectrum. Other malformation was also observed. Results Among 120 fetus, there were 119 cases with bilateral superior vena cava and 1 case with right superior vena cava interruption. There were 118 cases with left superior vena cava reflux to right atrium, and 2 cases with reflux to left atrium. There were 102 cases with simple persistent superior vena cava, accounting for 85.00%, 8 cases complicated with ventricular septal defect, accounting for 6.67%, 2 cases complicated with atrial septal defect, accounting for 1.67%, and 8 cases complicated with left ventricular dysplasia syndrome, accounting for 6.67%. Conclusion Early diagnosis for persistent superior vena cava, according to its characteristics, provides important significance. It can prevent occurrence of other congenital diseases.
