摘　　要：目的探讨住院老年糖尿病患者胰岛素注射行为强化指导干预的效果。方法便利抽样法选择2013年6月至2014年6月在上海交通大学医学院附属新华医院住院的2型糖尿病患者60例,按入院先后将60例患者分为观察组40例、对照组20例,对照组患者给予常规指导,观察组患者除常规指导外,还为每位患者提供指针式血糖监测-进食提示卡,按照提示卡时间准确进食。对2组患者从入院至出院后1个月进行随访,比较生化指标(空腹血糖、餐后血糖和糖化血红蛋白)和自我保健能力的变化。结果观察组低血糖发生率较对照组明显下降,差异有统计学意义(P<0.05)。出院后1个月,两组组患者的糖化血红蛋白、空腹及餐后血糖均较入院时有所下降,观察组较对照组下降更为显著;观察组的自我保健能力较对照组有更显著的提升。结论对于住院老年胰岛素注射糖尿病患者采取强化指导的方式进行干预,更能使强化指导在2型糖尿病的综合治疗中显示出其重要意义。Objective To discuss the effect of behavioral enhancement guidance in hospitalized elderly diabetes patients with injection of insulin.Methods A total of 60 cases were selected and randomly divided into observation group（n=40）and control group（n=20）.The control group received conventional guidance,while the observation group had pointer type of blood sugar monitoring-eating cue card on the basis of conventional nursing.The 1 month of follow-up was conducted on both two groups,the biochemical indexes（fasting blood-glucose,postprandial blood sugar and glycosylated hemoglobin）and changes of self-care ability were compared.Results The incidence of hypoglycemia was significantly reduced compared with that of control group（P〈0.05）.After 1 month of discharge,the fasting blood-glucose,postprandial blood sugar and glycosylated hemoglobin decreased in both two groups,and the indexes in observation group were significantly lower than that of control group.The self-care ability in observation group was significantly improved than that of control group.Conclusion The intervention of behavioral enhancement guidance in hospitalized elderly diabetes patients with injection of insulin can strengthen the significance of behavioral enhancement guidance in the comprehensive treatment of type 2 diabetes.
