睡前饮食对老年2型糖尿病患者夜间低血糖的干预效果

摘　　要：目的运用动态血糖监测系统,评价睡前饮食对老年2型糖尿病患者夜间低血糖发生率的干预效果。方法选择佩戴动态血糖监测系统且睡前（22：00监测）指尖血糖在5.0-5.6mmol/L之间的老年2型糖尿病患者100例,随机分为观察组和对照组,观察组给予饮食干预,对照组进行常规护理,比较两组患者夜间低血糖、高血糖发生率及血糖波动幅度。结果观察组夜间低血糖发生5人次,对照组发生44人次,差异有统计学意义（P〈0.01）;观察组夜间高血糖发生4人次,对照组发生2人次,差异无统计学意义（P〉0.05）;两组血糖波动幅度比较：观察组血糖范围在3.9-5.6mmol/L时各时间段持续时间比较差异有统计学意义（P〈0.05）;对照组血糖范围在≤3.9、3.9-5.6、5.7-10mmol/L时各时间段持续时间比较差异有统计学意义（P〈0.05）。结论老年2型糖尿病患者22：00血糖若低于5.6mmol/L时,应给予饮食干预,可有效预防夜间低血糖的发生,同时不增加高血糖风险,而且降低了血糖波动幅度。Objective Applying the dynamic glucose monitoring system to evaluate the intervention effect of bedtime diet on nocturnal hypoglycemia of elderly patients with type 2 diabetes.Methods A total of 100 cases wearing dynamic glucose monitoring system and whose fingertip blood glucose（at 22：00）was between 5.0-5.6mmol/L were selected and divided into observation group and control group.The observation group received diet intervention,while the control group received conventional nursing.The incidence of nocturnal hypoglycemia,hyperglycemia and glucose fluctuation range were compared.ResultsThere were 5 cases had nocturnal hypoglycemia in observation group,and 44 cases in control group（P〈0.01）.There were 4 cases of nocturnal hyperglycemia in observation group,and 2 cases in control group（P〉0.05）.The glucose fluctuation range in observation group was 3.9-5.6mmol/L（P〈0.05）,and 〈3.9,3.9-5.6,5.7-10mmol/L in control group（P〈0.05）.Conclusion There is a need of diet intervention if the blood glucose at 22：00 is lower than 5.6mmol/L,which can effectively prevent the incidence of nocturnal hypoglycemia without increasing the risk of hyperglycemia,and also reduce the glucose fluctuation range.
