基于保护动机理论的护理干预对2型糖尿病病人用药依从性及生活质量的影响

摘　　要：[目的]探讨基于保护动机理论(PMT)的健康教育模式对2型糖尿病病人用药依从性及生活质量的影响。[方法]采用便利抽样法选取某三级甲等医院内分泌科门诊就诊的2型糖尿病病人80例随机分为对照组及干预组,干预组实施基于PMT的护理干预,对照组病人实施一般护理措施及随访,比较两组病人血糖、糖化血红蛋白等实验室指标、用药依从性和生活质量。[结果]干预6个月后,干预组病人的血糖、糖化血红蛋白和餐后2h血糖低于对照组(P<0.05);干预12个月后,干预组病人用药依从性及生活质量的躯体健康、躯体角色功能、情绪角色功能得分高于对照组,躯体疼痛、健康变化指标得分低于对照组(P<0.05)。[结论]基于PMT的综合护理干预有助于帮助病人有效控制血糖,提高病人的用药依从性和生活质量。Objective：To probe into the influence of health education model based on protection motivation theory（PMT）on the medication compliance and quality of life of patients with type 2diabetes mellitus.Methods：Using convenience sampling method,a total of 80 cases of patients with type 2diabetes mellitus were selected from endocrinology clinic in a third grade A hospital and they were randomly divided into control group and intervention group.The patients in intervention group received nursing intervention based on PMT,the patients in control group received general nursing and follow-up,then to compare the laboratory indexes including blood glucose and glycated hemoglobin,and medication compliance and quality of life between both groups.Results：After 6months of intervention,the blood glucose,glycosylated hemoglobin and postprandial 2hblood glucose were lower than those in control group（P〈0.05）.After 12 months,the scores of medication compliance and physical health,physical role function and emotional role function of life quality in intervention group were higher than that in control group（P〈0.05）,the scors of body pain and health change index were lower than that in control group（P〈0.05）.Conclusion：The comprehensive nursing intervention based on PMT could help patients to control blood glucose effectively,improve the patient＇s medication compliance and quality of life.
