摘　　要：目的:探讨B超监测在宫腔镜手术中的临床应用价值。方法:对2005年1月至2014年12月我院在B超监测下经宫腔镜手术的病例进行回顾性分析,探讨B超监测在宫腔镜手术中的临床应用价值。结果:子宫肌瘤电切术267例,其中黏膜下肌瘤243例,壁间肌瘤内突大于50%有24例,术后月经量均明显改善,内突型壁间肌瘤患者残余肌瘤无增大;宫腔黏连分解术179例,1次完成133例,2次手术分粘46例,月经恢复率100%,有生育要求的67例中,术后妊娠46例,妊娠率68.7%(46/67);子宫纵隔切除术29例,手术均1次完成;475例手术无子宫穿孔等并发症发生。结论:B超监测引导下实施宫腔镜手术是一种简便、经济、安全、有效的监护方法,是手术成功的安全保障,可作为宫腔镜手术的首选监护方法推广应用。Objective： To assess the clinical application of B-ultrasonic monitoring during hysteroscopy surgery. Methods： The data of B-ultrasonic monitored hysteroscopy surgery in Affiliated Hospital of North Sichuan Medical College from Jan 2005 to Dec 2014 was retrospectively analyzed. Results： There are 267 patients underwent TCRM（ transcervical resection of myoma）,including 243 patients with submucous myoma and 24 with intramural myoma. The menstrual flow volume has been decreased obviously after surgery.There are 179 patients with intrauterine adhesions decomposition,of which 133 cases experienced successful hysteroscopy surgery under B-ultrasound supervision,while 46 cases have been tried twice. The rate of menstruation recurrence was 100%. Among 67 patients with fertility requirements,46 of them got pregnancy（ 68. 6%,46 /67）. Meanwhile,twenty-nine cases with mediastinal resection of uterus had been treated by hysteroscopy surgery under monitor by ultrasound. There were no severe complications like uterine perforation in total 475 cases of operations. Conclusion： Hysteroscopy surgery monitored by B-ultrasound is a simple,economical,safe and effective approach to guarantee the success of operation and worthy of promotion with priority.
