摘　　要：立足合肥市社会工作介入居家养老服务的项目化实践,提出"政府+社会工作服务社"的居家养老层级化运作框架。社会工作以项目化的服务方式介入居家养老具有三大特殊优势:提供个案管理式综合养老服务、设计多元化服务项目为社区老人匹配品牌化服务、有力整合和使用社区内外养老服务资源。持续推进社会工作介入居家养老服务应该从强化社区为老服务教育、倡导"自助互助"式养老、加强社工机构能力建设、完善政策法规支持等方面着手。This paper,based on the practice of the intervention of social work in home-based care services in Hefei City,presents the concept of 'government + social work services',a hierarchical operational framework of home-based care. The practice shows that the intervention of social work in home-based care services has three peculiar advantages: to offer a comprehensive case management service for the aged,design various service items to furnish branded service to the aged in the community,and integrate the service resources for the aged from the inside and outside of community. To constantly promote the intervention of social work in home-based care services,it' s necessary to first strengthen the education on community services for the elderly,advocate 'self-service and mutual-help'cares for the aged,enhance the construction of social service organization,and improve policy and legal support.
