小儿先天性复拇指畸形170例WASSEL分型及手术治疗

摘　　要：目的总结复拇指畸形的临床WASSEL分型,明确手术治疗方法的优点。方法对2014-01/2015-01本溪钢铁总医院骨科收治的170例复拇指畸形病例进行回顾性分析。通过皮肤切口的设计、骨骼支架的建立、关节的修复、拇外展功能的重建等方法,对小儿先天性复拇指畸形进行手术治疗。结果手术后得分4~5分100例,3分65例,2分以下5例。170例术后拇指的外形及功能均取得满意的效果。随访6~36个月,除去2例侧弯畸形、3例关节运动受限、6例患指较健侧肥大外,全部经康复锻炼后,基本上满足日常生活需要。结论先天性多拇指畸形的治疗,根据WASSEL分型选择适当手术时机,可取得较好的疗效。Objective To sum up the clinical WASSEL classification of complex thumb,and to clarify the advantages of surgical treatment.Methods A retrospective analysis was made in 170 cases of complex thumb deformity.Through the skin incision design,skeleton establishment,joint repair and functional reconstruction of thumb abduction,congenital thumb deformities were treated surgically.Results The score of 4-5after operation were in 100 cases,3points in 65 cases and under 2points in 5cases.The shape and function of the thumb were satisfactory in 170 cases.After a follow-up of 6-36 months,except 2cases of scoliosis,3cases of joint movement limitation and 6cases with fingers bigger than the healthy one,all basically met the needs of daily life after rehabilitation training.Conclusion In the treatment of congenital thumb deformities,choosing appropriate surgery time according to WASSEL classification can achieve good effect.
