孟鲁司特钠治疗小儿急性上呼吸道感染后喘息的作用

摘　　要：目的观察孟鲁司特钠对小儿急性上呼吸道感染后喘息的预防作用。方法 2013-09/2014-08广州中医药大学祈福医院儿科收治1~3岁既往有2次或以上喘息的急性上呼吸道感染患儿126例,随机分为两组,观察组66例,对照组60例。在予对症治疗基础上,观察组于急性上呼吸道感染发病24h内予口服孟鲁司特钠4mg,每日1次,连服10d,对照组则于急性上呼吸道感染出现喘息后予口服孟鲁司特钠片4mg,每日1次,连服10d。停药3个月后再次出现符合诊断标准的急性上呼吸道感染则根据交叉原则进行交叉对照。观察两组出现喘息的例数及重度喘息的例数。结果观察组发生喘息的例数及重度喘息的例数于交叉前均较对照组明显减少,于交叉后则均较对照组明显增多。结论孟鲁司特钠对1~3岁小儿急性上呼吸道感染后喘息有预防作用,早期应用可减少喘息发作的机会及减轻发作的程度。Objective To observe the prevention effect of montelukast sodium on wheezing in children with acute upper respiratory tract infection.Methods Totally 126 cases of children of 1-to 3-year-old with previously wheezing attack for more than two times after acute upper respiratory tract infection were divided into two groups：66cases for A group,60 cases for B group.After basic symptomatic treatment,children in A group were treated with oral montelukast sodium at 4mg once daily within 24 hours after the disease attack,and the treatment lasted for 10 days.Children in B group were treated with oral montelukast sodium at 4mg once daily after the appearance of wheezing symptom,and the treatment also lasted for 10 days.According to cross-over principle,we performed cross comparison of the two groups if children were diagnosed with acute upper respiratory tract infection again 3months after stopping taking montelukast sodium.The cases of wheezing were observed and recorded.Results The number of children in A group with wheezing and serious wheezing was lower than that in B group before cross comparison.However,after cross comparison,the number of children with wheezing and serious wheezing in A group was higher than that in B group.Conclusion Montelukast sodium has preventive effect on wheezing in children between1-to 3-year-old with acute upper respiratory tract infection.Early application can reduce the rate of wheezing attack and alleviate the severity.
