摘 要:目的研究应用KalixⅡ治疗有临床症状的青少年柔韧性平足症患者,并对其临床疗效进行临床和影像学综合评价。方法回顾性分析2008年1月至2012年6月在我科接受应用KalixⅡ制动器行距下关节制动术治疗的19例(25足)有临床症状的青少年柔韧性平足症患者,男12例,女7例;平均年龄12.2岁。应用AOFAS评分、VAS评分和测量负重X线片对术前、术后情况进行评价。结果 19例患者均获得随访,随访时间平均23.3个月。AOFAS评分由术前平均66.6分提升为术后88.1分,VAS评分由术前平均6分降至术后1分。足外形较术前明显改善。前后位片上第1距角由术前平均18.7°改善为术后6.2°,侧位片第1跖距角由术前19.2°改善为术后4.5°,侧位片跟骨倾斜角由术前8.5°改善为术后12.7°。结论应用Kalix II制动器行距下关节制动术是一种治疗有临床症状的青少年柔韧性扁平足的有效方法,其远期疗效尚需进行多中心大样本的研究和长期观察。Objective To explore the efficacies of Kalix Ⅱ in juvenile flexible flatfoot. Methods From January 2008 to June 2012, retrospective analyses were conducted for 19 flexible flatfoot cases(25 feet) undergoing subtalar arthroereisis. We measured the angles and used the AOFAS score and VAS score to evaluate and analyze the preoperative and postoperative status. Results During a follow-up period of 23,3 months. Postoperatively foot pain, arch collapse and other symptoms obviously improved. The mean AOFAS scores rose from preoperative 66.6 points to postoperative 88.1 points (P〈0.05). On anteroposterior X-ray films, talar-first metatarsal angles decreased from preoperative 18.7° to postoperative 6.2° (P 〈0.05), on lateral X-ray films, talar-first metatarsal angles decreased from preoperative 19.2° to postoperative 4.5°(P 〈0.05) while calcaneal angles increased from preoperative 8.5° to postoperative 12.7° (P 〈0.05). Conclusions Subtalar arthroereisis use KalixII is a simple and effective way to treat juvenile flexible flatfoot. Further studies and long-term studies are needed before these devices can be recommended for general use.
