摘  要:目的探讨腺病毒E1A蛋白在慢性阻塞性肺疾病(COPD)患者肺组织内的表达情况及与血清C反应蛋白(CRP)水平的相关关系。方法收集因肺癌手术切除远端的正常肺组织22例及行肺减容手术的COPD患者的肺组织石蜡包埋标本46例。采用免疫组织化学法检测各组肺组织内腺病毒E1A蛋白的表达,用IPP软件对肺组织切片进行图像分析;所有入组患者均在手术前常规抽血,采用免疫比浊法对血清CRP水平进行测定。结果对照组血清CRP水平为(5.67±0.37)mg·L-1,COPD组血清CRP水平为[轻度(12.45±1.05)mg·L-1、中重度(18.25±3.30)mg·L-1],两两比较差异均有统计学意义(P均<0.05);血清CRP水平与FEV1的实测值与预测值的百分比(FEV1%pred)呈负相关(r=-0.812,P<0.01);腺病毒E1A蛋白在肺泡上皮细胞、气道上皮细胞中均有表达;中重度COPD组腺病毒E1A蛋白的阳性表达率高于对照组(χ2=4.330,P=0.037);COPD组腺病毒E1A蛋白的阳性表达率与FEV1%pred呈负相关(r=-0.405,P=0.028);COPD组E1A蛋白免疫染色强度为[轻度(2.06±0.75)分、中重度(2.56±0.73)分],与对照组(0.68±0.82)分相比明显增强(P均<0.05);COPD组E1A蛋白表达强度与血清CRP水平呈正相关(r=0.692,P<0.01)。结论 COPD患者腺病毒E1A蛋白的阳性表达率较高,并与肺功能分级及血清CRP水平呈正相关,腺病毒E1A蛋白可能参与COPD气道慢性炎症的发生发展。Objective To explore the expression of adenovirus E1 A protein in lung tissues of chronic obstructive pulmonary disease( COPD) patients and its correlation of adenovirus E1 A protein with the level of serum C- reactive protein( CRP). Methods Tissues from the patients with COPD( n = 46) was obtained by lung volume reduction surgery. Tissues from the patients with normal lung function( n = 22) was obtained from lung lobes resected for tumor. Blood samples were collected from all participants. The expression of adenovirus E1 A protein in lung tissue was detected by immunohistochemical method. imaging analysis of specimens were done by Image- Pro Plus( IPP) software and the level of serum C- reactive protein was measured by immunoturbidimetry method. Results The serum level of CRP increased significantly in COPD groups compared with the control subjects( P〈0. 05). The serum level of CRP and FEV1 % pred showed significant negative correlation( r =- 0. 812,P〈0. 812). Adenovirus E1 A protein presented in alveolar epithelium,epithelium of the conducting airway and bronchial glands. E1A- containing cells increased significantly in severe COPD compared with control subjects( χ^2= 4. 330,P = 0. 037) and this increase in E1A- containing cells was significant negative correlation with FEV1 % pred( r =- 0. 405,P = 0. 028). The immune staining intensity of adenovirus E1 A protein increased significantly in severe COPD compared with control subjects( P〈0. 05).There was a significantly positive correlation between the proportion of E1A- containing cells and the level of serum CRP( r = 0. 692,P〈0. 01). Conclusion E1A- containing cells increased in severe COPD compared with control subjects. This increase in E1A- containing cells was significant negative correlation with reduction in FEV1 % pred and positive correlation with the serum CRP level. Adenovirus E1 A protein may play an important role in the development of COPD chronic airway inflammation.
