摘  要:目的探讨多层螺旋CT时间减影血管造影(Neuro-DSA)在颈动脉狭窄诊断中的应用价值。方法回顾性分析30例疑似颈动脉病变患者的Neuro-DSA图像,以颈动脉数字减影血管造影(DSA)结果作为对照,分析并比较两种方法诊断血管狭窄程度的一致性。结果 1Neuro-DSA与DSA对30例患者180支血管的检出结果比较,差异无统计学意义(χ2=1.11,P>0.05)。2以DSA为金标准,Neuro-DSA诊断血管无狭窄、轻度、中度、重度狭窄和闭塞的准确度分别为94.4%、94.4%、92.2%、97.8%和100.0%。Neuro-DSA与DSA诊断一致性的Kappa值为0.65(P<0.01)。3Neuro-DSA有效剂量(2.58±0.27)m Sv低于DSA(330.00±20.00)m Sv(P<0.01)。结论 Neuro-DSA能较清楚、直观地显示颈动脉狭窄位置、程度,尤其在诊断血管重度狭窄和闭塞方面具一定优势。Objective To explore the Clinical value of MSCT Neuro- DSA in the diagnosis of carotid artery stenosis. Methods MSCT Neuro- DSA images of 30 patients suspected carotid artery disease were analyzed respectively,and compared with the results of carotid DSA. The image quality were evaluated. Results 180 vessels met the diagnostic requirements,sensitivity,specificity,accuracy for detecting diagnosis of carotid artery stenosis were as following: 92. 0%,96. 7%,94. 4% for without stenosis; 93. 0%,94. 9%,94. 4% for mild stenosis; 78. 1%,95. 3%,92. 2%,0. 81 for moderate stenosis; 85. 7%,98. 8%,97. 8% for severe stenosis; 100. 0%,100. 0 %,100. 0% for occlusion. Conclusion MSCT Neuro- DSA can more clearly display the location,extent of carotid stenosis and it has some advantages in diagnosis of carotid artery disease.
