摘 要:目的研究酪蛋白磷酸肽-无定形磷酸钙(casein phosphopeptide-amorphous calcium phosphate,CPP-ACP)体外再矿化脱矿牙本质的效能和机制。方法 1制备牙本质片20片,37%磷酸脱矿30s,随机数字表法分为2组,分别经人工体液(simulated body fluid,SBF)和CPP-ACP处理15 d之后进行显微硬度测定、表面形貌观察及X线衍射分析。2构建胶原纤维镍钛网,采用透射电镜观察CPP-ACP和SBF处理的重组胶原,并进行选区衍射分析。结果处理15 d后CPP-ACP组和SBF组牙本质表面显微硬度分别为(47.92±4.25)和(42.72±4.65)维氏硬度值,CPP-ACP组显著高于SBF组(P〈0.05),且扫描电镜见CPP-ACP组牙本质表面较SBF组明显致密,有大量以羟基磷灰石为主的沉积物生成。透射电镜见CPP-ACP组重组胶原纤维上及周围分布有密集呈梭形的长50-60 nm的无定形磷酸钙。SBF组仅见少量类圆形无定形磷酸钙。结论 CPP-ACP可促进离体脱矿牙本质再矿化,表面形成以羟基磷灰石为主的矿物质沉积,可用于根面龋和牙本质过敏的非手术治疗。Objective To investigate the effect and mechanism of casein phosphopeptide-amorphous calcium phosphate( CPP-ACP) on the remineralization of demineralized dentine,and provide reference for the prevention or non-invasive intervention of root caries and dentine hypersensitivity. Methods Twenty dentine specimens demineralized with 37% phosphoric acid gel for 30 s were randomly divided in 2 groups with a random digital table. Then the specimens were treated with simulated body fluid( SBF) and CPP-ACP,respectively, for 15 d. The dentine surfaces were subjected to microhardness measurement,micromorphological evaluation and X-ray diffraction analysis. Collagen fiber nickel-titanium meshes were constructed and treated with SBF and CPP-ACP for 1 d. Then,the meshes were subjected to micromorphological observation and selected area diffraction analysis by transmission electron microscopy. Results The Vickers hardness values of the dentine surfaces in the CPP-ACP group and SBF group were 47. 92 ± 4. 25 and 42. 72 ±4. 65,respectively,and the microhardness in the CPP-ACP group was significantly higher than that in the SBF group after treatment for 15 d( P〈0. 05). The dentine surface in the CPP-ACP group was more compact than that in the SBF group,and many sediments were generated on the dentine surface and identified mainly as hydroxylapatite( HA). Lots of 50 - 60 nm fusiform ACP was distributed on the recombinant collagen fibers and the periphery in the CPP-ACP group,and only a few of round ACP was observed in the SBF group by transmission electron microscopy. Conclusion CPP-ACP can promote in vitro remineralization of demineralized dentine,and generate HA as main minerals on the dentine surface. So it can be used for the non-operative treatment of root caries and dentin hypersensitivity.
