胎儿系统超声筛查在湖北省各级医院中的推广应用

摘　　要：目的探讨在湖北省基层医院推广普及与实施胎儿系统超声筛查规范的培训模式。方法应用统一的胎儿系统超声筛查标准,统一培训教材、培训方法和筛查质量控制,通过举办长期骨干培训班与短期基层培训班相结合的方式,对湖北省内行产前超声检查的医师进行培训。结果 2005-2012年间完成长期骨干培训班15期,培训医院152家,人员189名,其中来自湖北省的医院有65家,学员有82名;完成湖北省内短期基层培训班11期,培训医院466家,人员1 159名。长期骨干培训班湖北省学员中87.7%来自二甲及以上医院,其中三级医院占27.7%;短期基层培训班中91.0%为二甲及以下医院,其中一级医院占52.2%。长期骨干培训班湖北省学员职称分布为:高级职称14.6%,中级职称65.9%,初级职称及其他19.5%;短期基层培训班学员职称分布为:高级职称12.2%,中级职称36.6%,初级职称及其他51.2%。规范统一了湖北省胎儿系统超声筛查标准、培训教材、培训方法,统一了湖北省产前筛查机构超声人员准入标准、质量控制标准及产前超声筛查项目的标准。建立了网络交流互动平台。结论城市与基层培训相结合、短期理论培训与长期理论加实际训练相结合、互补不足可作为在基层医院推广普及与实施胎儿系统超声筛查规范培训模式。Objective To explore the promotion, popularization, and implementation of standard training mode of fetal systemic ultra- sound screening in basic-level hospitals of Hubei province. Methods Using unified fetal systemic ultrasound screening standard, unified training material, training method, and quality control for screening, the training was carried out for the doctors who performed prenatal ultrasound examination in Hubei province through combination of long-term backbone training class and short-term training course at basic level hospitals. Results From 2005 to 2012, 15 rounds of long-term backbone training courses were completed for the 189 participants from 152 hospitals, including 82 participants from 65 hospitals from Hubei province, 11 rounds of short-term basic-level training courses were com- pleted for 1 159 participants from 466 hospitals in of Hub＇el province. In long-term backbone training courses, 87.7% of Hubei native participants were from second-grade class-A hospitals or above, including 27.7% of respondents from third-grade hospitals; ,in short-term basic-level training courses, 91.0% of the participants were from second-grade class-A hospitals or below, including 52. 2% of respondents from first-grade hospitals. In long-term backbone training courses, title distributions of Hubei native participants were： 14. 6% senior, 65.9% intermediate and 19. 5% others; in short-term basic-level training courses, title distributions of Hubei native participants were： 12.2% senior, 36. 6% intermediate and 51.2% junior or others. The standard, training material, and training method of fetal systemic ul- trasound screening in Hubei province were normalized and unified, and inclusion criteria for prenatal ultrasound screening staff, quality control standard, and standard of prenatal ultrasound screening in Hubei province were unified.
