腹腔镜下输卵管壶腹部妊娠切开取胚术的治疗效果

摘　　要：目的为了进一步提高输卵管壶腹部妊娠的临床治疗效果和保留患者的生育功能,分析和探讨采取腹腔镜下输卵管壶腹部妊娠切开取胚术可行性和必要性。方法选取280例于2011年1月-2015年6月共280例输卵管壶腹部妊娠病例作为研究对象,对照组(200例)和观察组(80例),其中观察组患者治疗上采取腹腔镜下输卵管壶腹部妊娠切开取胚术,对照组患者在治疗方案上采取腹腔镜下患侧输卵管切除术,对比和分析两组患者治疗效果和术后患者血中β-hC G的数值变化。结果两组的临床治疗效果和术后患者血中β-hC G的数值变化相比较,无统计学差异(P>0.05)。结论腹腔镜下输卵管壶腹部妊娠切开取胚术,在疗效相同情况下保留了输卵管,让患者有继续自然怀孕的机会,对有生育要求的患者是一种比较适合的手术方式。Objective To analyze and explore the feasibility and necessity of laparoscopic surgery incision for tubal ampullary pregnan- cy, further improve the clinical curative effect of tubal arnpullary pregnancy and conserve reproductive function of the patients. Methods A total of 280 eases with tubal ampullary pregnancy were selected from January 2011 to June 2015 as study object, then they were divided into control group （200 eases） and observation group （80 cases） . The cases in observation group were treated by laparoscopic surgery incision for tubal ampullary pregnancy, while the cases in control group were treated by affected side salpingectomy under laparoscope. The curative effects and the chauges of blood β-human chorionic gonadotropio （ β-hCG ） levels after treatment in the two groups were compared and ana- lyzed. Results There was no statistically significant differeuce in the clinical curative effect and the change of blood [？,-hCG level after treatment between the two groups （P〉0. 05） . Conclusion Laparoscopic surgery incision for tubal ampullary pregnancy can conserve Fallopian tubes with the same curative effect compared with affected side salpingectomy under laparoscope, the patients have the opportunity to continue spontaneous pregnancy, which is a suitable surgical method for the patients having birth requirement.
