摘  要:目的 观察芬太尼全身麻醉术中应用地佐辛对行妇科手术患者血流动力学及术后恢复的影响。方法 ASA分级Ⅰ~Ⅱ级、择期行妇科手术患者30例,随机分为地佐辛组和对照组各15例,地佐辛组切皮后30 min给予地佐辛0.15mg/kg,对照组不应用地佐辛及其他药物。记录2组麻醉诱导前、手术开始、手术1h、术毕即刻平均动脉压(mean arterial pressure,MAP)、心率;记录2组手术时间、芬太尼总量、术后拔管时间、苏醒时间、意识恢复时间及拔管前后躁动发生情况。结果 2组麻醉诱导前、手术开始、手术1h、术毕即刻MAP、心率及手术时间、芬太尼总量、术后拔管时间、苏醒时间及意识恢复时间比较差异无统计学意义(P〉0.05);地佐辛组拔管前后躁动发生率(6.7%)明显低于对照组(26.7%),差异有统计学意义(P〈0.05)。结论 芬太尼全身麻醉下行妇科手术患者切皮后30min应用地佐辛可维持术中血流动力学平稳,拔管前后躁动发生率低。Objective To observe the influence of dezocine in fentanyl general anesthesia on hemodynamics and recovery after gynecologic operation. Methods Thirty gynecological patients with ASA Ⅰ to Ⅱ were randomly divided into dezocine group receiving 0.15 mg/kg within 30 minutes after skin incision and control group not receiving dezocine, with 15 patients in each group. The main arterial pressure and heart rate were detected before anesthesia induction, at beginning of operation, 1 hour after skin incision, and at the end of the operation. The operation lasting time, fentanyl amount, extubation time, awake time, consciousness recovery time and agitation were recorded before and after extubation. Results There were no significant differences in main arterial pressure and heart rate at each time point, the extubation time, awake time, and consciousness recovery time between two groups (P〉0.05). The incidence of agitation in dezocine group (6. 7%) was significantly lower than that in control group (26. 7%) after extubation (P〈0.05). Conclusion The administration of dezocine within 30 minutes after skin incision can effectively maintain the stable hemodynanics and reduce the incidence of agitation after extubation.
