可冲洗式持续负压引流在烧伤科治疗难治性创面修复中的应用

摘　　要：目的探讨可冲洗式持续负压引流在烧伤科治疗难治性创面修复中的应用及效果。方法回顾性分析2012年1月至2014年1月我科应用可冲洗式持续负压引流治疗的难治性创面病例资料共32例,其中电烧伤2例,四肢烧、烫伤4例,糖尿病足坏疽8例,顽固性溃疡12例,臀部褥疮2例。创面彻底清创止血,放置可冲洗式持续负压引流(VSD–B-2)引流7-21天,肉芽组织生长后二期创面植皮26例,皮瓣修复6例,直接缝合2例。结果本组32例患者,二期术后创面愈合良好。26例植皮患者,植皮成活率95%以上,6例皮瓣移植患者,皮瓣成活良好,无皮瓣下积血、皮瓣坏死、感染等并发症。随访6个月,功能、外形满意。结论采用可冲洗式持续封闭负压引流技术使创面渗液、渗血得以充分引流、促进肉芽组织生长,更好地收缩创面,为各种难治性、复杂性或特殊的创面修复提供一个很好的治疗手段,创面操作简单,疗效确切可靠,安全方便,值得推广。Objective To evaluate the application and efficacy of flush-type continuous negative pressure grainage on refractory wound repair in department of burn. Methods 32 patients in our hospital burn unit in January 2012 to January 2014 application can flush continuous negative pressure wound drainage in the treatment of various refractory cases were analyzed ： two cases in which the electrical burn, burn limbs, 4 cases of burns, diabetic foot gangrene eight cases, 12 cases of intractable ulcers, bedsores two cases buttocks. Bleeding wound debridement, placement can flush continuous negative pressure drainage （VSD-B-2） drainage 7-21 days after the growth phase of wound granulation tissue grafting in 26 cases, flap 6 cases, direct suture two cases. Results The group of 32 patients, two postoperative wound healed well. 26 cases of patients with skin grafting, graft survival rate of 95%, six cases of patients with flap, flap survived well, blood clots without flap, flap necrosis, infection and other complications. Followed up for six months, function, appearance satisfaction. Conclusion can flush technology enables continuous vacuum assisted closure wound exudate, bleeding can be adequate drainage, and promote the growth of granulation tissue, wound contraction and better, for a vadety of refractory, complexity, or provide a special wound repair good treatment, simple operation wounds and effective, reliable, safe and convenient, worthy of promotion.
