摘 要:目的探讨医院ICU重症感染患者病原菌耐药性及对Smad通路蛋白的影响,为临床治疗提供参考。方法选取医院2012年5月-2014年8月诊治的190例ICU重症感染患者设为感染组,以医院ICU 180例未感染患者为未感染组,同期180名健康体检者为对照组;分析感染患者感染病原菌分布及耐药性,对比3组患者血清中Smad通路蛋白的水平变化。结果 190例医院ICU重症感染患者标本分离出病原菌200株,其中革兰阳性菌85株占42.5%,革兰阴性菌115株占57.5%;革兰阳性菌对红霉素、头孢西丁、青霉素、四环素及阿莫西林耐药性较强,但对万古霉素较敏感;革兰阴性菌对头孢唑林、头孢呋辛、头孢西丁及哌拉西林和庆大霉素耐药性较强,但对亚胺培南均较敏感;感染组患者血清中Smad1、Smad2及Smad3水平明显高于健康对照组和未感染组患者,差异有统计学意义(P<0.01)。结论医院ICU重症感染患者耐药菌感染多发,且感染后患者血清中Smad蛋白水平明显升高。OBJECTIVE To explore the drug resistance of the pathogens isolated from the ICU patients with severe infections and observe the impact on Smad pathway protein so as to provide guidance for clinical treatment.METHODS A total of 190 ICU patients with severe infections who were treated from May 2012 to Aug 2014 were chosen as the infection group,180 ICU patients without infections were set as the non-infection group,and 180 healthy people who received physical examination during the same period were assigned as the control group.The distribution and drug resistance of the pathogens causing the infections were observed,and the level of serum Smad pathway protein was compared among the three groups.RESULTS Totally 200 strains of pathogens were isolated from the specimens obtained from 190 ICU patients with severe infections,including 85(42.5%)strains of gram-positive bacteria and 115(57.5%)strains of gram-negative bacteria.The gram-positive bacteria were highly resistant to erythromycin,cefoxitin,penicillin,tetracycline,and amoxicillin but were susceptible to vancomycin;the gram-negative bacteria were highly resistant to cefazolin,cefuroxime,cefoxitin,piperacillin,and gentamicin but were susceptible to imipenem.The levels of serum Smad1,Smad2,and Smad3 of the infection group were significantly higher than those of the control group and non-infection group(P〈0.01).CONCLUSIONThe severe infections caused by the drug-resistant pathogens are prevalent among the ICU patients,and the level of serum Smad protein of the patients with the infections is remarkably elevated.
