幽门螺杆菌感染与反流性食管炎的相关性评价

摘　　要：目的探讨幽门螺杆菌(Hp)感染与反流性食管炎(RE)的临床相关性,为RE发病机制的研究提供参考。方法随机选择2014年2月-2015年1月在医院消化内科接受检查的RE患者100例作为观察组,选取同期排除RE的人群100例作为对照组;比较不同内镜分级和不同临床症状程度的RE患者的Hp感染率及两组患者Hp感染率;采用SPSS19.0软件对数据进行统计分析。结果内镜分级A、B、C、D级RE患者Hp感染率分别为41.5%、37.5%、26.3%和25.0%,不同分级感染率相比,差异有统计学意义(P<0.05);症状轻度患者Hp感染率明显高于中度和重度患者,差异有统计学意义(P<0.05);观察组与对照组Hp感染率相比,差异无统计学意义。结论 Hp感染与RE患者内镜分级和症状严重程度呈负相关,可能在RE进展过程中起到保护作用。OBJECTIVE To explore the clinical correlation between the Helicobacter pylori infections and the reflux esophagitis so as to provide references for the research of pathogenesis of the reflux esophagitis.METHODS A total of 100 patients with reflux esophagitis who received examinations in the department of gastroenterology from Feb2014 to Jan 2015 were randomly selected as the observation group,and 100 individuals who were excluded the reflux esophagitis were set as the control group.The incidence of H.pylori infections in the patients who varied in the endoscope grading and the degree of clinical symptom was observed,and the incidence of H.pylori infections was compared between the two groups of patients;the statistical analysis of data was performed with the use of SPSS19.0software.RESULTS The incidence of H.pylori infections was 41.5%in the reflux esophagitis with the grade A endoscope,37.5% in the patients with grade B endoscope,26.3% in the patients with grade C endoscope,25.0%in the grade D endoscope,and there was significant difference（P〈0.05）.The incidence of H.pylori infections was significantly higher in the mild symptom patients than in the moderate and severe symptom patients（P〈0.05）;there was no significant difference in the incidence of H.pylori infections between the observation group and the control group.CONCLUSIONThe H.pylori infections are negatively correlated with the endoscope grading and severity of symptom of the patients with reflux esophagitis,which may play aprotective role in the progression of reflux esophagitis.
