女性外阴尖锐湿疣人乳头状瘤病毒感染的相关性研究

摘　　要：目的对女性外阴尖锐湿疣人乳头状瘤病毒(HPV)临床感染患者的相关性问题进行研究,以提高女性外阴尖锐湿疣人乳头状瘤病毒感染患者诊断的准确性和治疗针对性。方法选择2013年6月-2014年5月医院接诊的76例女性外阴尖锐湿疣患者进行研究,按照感染程度的不同将高危型感染患者设为对照组,共36例,低危型感染患者为观察组,共40例;对患者检测HPV阳性标本基因分型构成进行分析,并跟踪调查两组患者出院后3个月和6个月的HPV基因分型复发率。结果两组患者HPV阳性标本基因分型构成比比较,观察组检测有6、53、11型和6+11型4种分型,对照组有24、8、10型3种分型,HPV阳性标本基因分型构成对比,差异有统计学意义(P<0.05);观察组患者出院后3个月HPV基因分型复发率为20.00%,对照组为16.67%,两组比较差异无统计学意义;患者出院6个月后,观察组患者HPV基因分型复发仅4例,远低于对照组的11例,其复发率也显著降低,为10.00%,低于对照组的30.56%,差异有统计学意义(P<0.05)。结论由于不同女性外阴尖锐湿疣患者其HPV基因分型不同,导致不同患者HPV感染复发率也不尽相同;因此,在实际临床诊疗中,应根据不同的HPV基因分型给予相应的治疗。OBJECTIVE To study the issues related to the clinical human papillomavirus（HPV）infections in female patients with genital warts so as to improve the accuracy of diagnosis of the genital warts patients with human papillomavirus infections and conducted targeted treatment.METHODS A total of 76 female patients with genital warts who were treated in the hospital from Jun 2013 to May 2014 were enrolled in the study and were divided into the control group with 36cases（with high-risk infection）and the observation group with 40cases（with low-risk infection）according to the severity of the infections.The genotypes of the specimens tested positive for HPV were observed,and the follow-up was conducted to investigate the recurrent rates of HPV genotypes of the two groups of patients who were discharged for 3and 6months.RESULTS As compared with the genotypes of the HPV-positive specimens,there were 4genotypes（type 6,53,11,and 6plus 11）in the observation group and 3genotypes（type 24,8,and 10）in the control group,with statistical significant（P〈0.05）.The recurrence rate of the HPV genotype was 20.00%in the observation group after the discharge for 3months,16.67%in the control group,and there was no significant difference between the two groups.The recurrence rate of the HPV genotype was10.00%（4cases）in the observation group after the discharge for 6months,significantly lower than 30.56%（11cases）in the control group（P〈0.05）.CONCLUSION The recurrence rate of HPV infection varies among the female patients with genital warts due to the different genotypes of HPV,therefore,it is necessary to conduct the corresponding treatment in the clinical practice according to the genotypes of HPV.
