剖宫产术中不同温度甲硝唑溶液冲洗宫腔对产后感染的影响分析

摘　　要：目的探讨剖宫产术中不同温度甲硝唑溶液冲洗宫腔对产妇产后感染的影响。方法选取医院2012年1月-2015年1月收治的行剖宫产高危感染产妇160例随机分为4组,每组各40例,对照组产妇给予生理盐水冲洗,A、B、C组给予22~24℃、39~40℃、37.5~37.8℃不同温度甲硝唑溶液冲洗,比较4组产妇围手术期相关参数与术后结局;采用SPSS19.0软件对数据进行统计分析。结果 4组产妇产后发热时间、排气时间与下床时间差异有统计学意义（P〈0.01）;C组产后发热时间显著低于A组,差异有统计学意义（P〈0.01）;4组产妇宫腔感染率与产褥病发生率比较差异有统计学意义（P〈0.01）;C组产褥病发生率明显低于A、B组,差异有统计学意义（P〈0.05）。结论剖宫产术中使用甲硝唑溶液冲洗宫腔可有效缩短产后发热时间、排气时间与下床时间,预防宫腔感染与减少产褥病,而甲硝唑溶液在37.5~37.8℃时效果最显著。OBJECTIVE To explore the effect of intraoperative uterine flushing with metronidazole solution under different temperatures on postpartum infections in the puerpera undergoing cesarean section.METHODS A total of160 puerpera with high-risk infections who underwent the cesarean section in the hospital from Jan 2012 to Jan2015were enrolled in the study and randomly divided into four groups,with 40 cases in each.The control group was treated with normal saline for the flushing,while the group A,B,and C groups were treated with metronidazole solution for the flushing under different temperatures 22~24℃,39~40℃,and 37.5~37.8℃.The related perioperative indexes and postoperative outcomes were observed and compared among the four groups,and the statistical analysis of data was performed with the use of SPSS19.0software.RESULTS There was significant difference in the postpartum fever time,exhaust time or discharge time among the four groups（P〈0.01）;the postpartum fever time of the group C was significantly lower than that of the group A,and there was significant difference（P〈0.01）.There was significant difference in the incidence of intrauterine infections or puerperalism among the four groups（P〈0.01）;the incidence of puerperalism was significantly lower in the group C than in the group A and B（P〈0.05）.CONCLUSIONThe intraoperative uterine flushing with metronidazole solution can effectively shorten the postpartum fever time,exhaust time,and ambulation time of the puerpera undergoing the cesarean section and reduce the incidence of intrauterine infections and puerperalism.The effect of metronidazole solution is most remarkable when the temperature varies from 37.5to 37.8 ℃.
