摘 要:目的探讨手卫生综合干预对肿瘤内科患者医院感染的影响,以有效预防患者医院感染。方法选取医院肿瘤内科医护人员为研究对象,自2013年6月开始实施手卫生综合干预,分析干预前(2011年10月-2013年5月)与干预后(2013年6月-2015年1月)医护人员手卫生执行率、患者感染情况、手消毒用品每床日消耗量、手微生物合格率及感染部位;采用SPSS16.0软件对数据进行统计分析。结果干预前肿瘤内科住院患者感染率为9.67%,干预后感染率为4.67%,明显低于干预前(P<0.01);干预前后长期住院肿瘤患者的感染部位均以上呼吸道、下呼吸道、消化道、泌尿道居多,其感染率分别为3.67%、3.33%、3.67%、5.33%和2.33%、2.33%、2.67%、2.33%,干预前后比较差异有统计学意义(P<0.05);干预前后手卫生执行率、手消毒用品及手微生物合格率比较,差异均有统计学意义(P<0.05)。结论手卫生综合干预可提高医护人员手卫生执行率,提高手微生物合格率,有效降低肿瘤内科患者医院感染率。OBJECTIVE To explore the influence of comprehensive intervention to hand hygiene on nosocomial infections in patients of oncology department so as to effectively prevent the nosocomial infections.METHODS The health care workers of the oncology department were recruited as the study objects,the comprehensive intervention to hand hygiene was carried out since Jun,2013;the implementation rate of hand hygiene,prevalence of infections in patients,daily consumption of hand disinfection supplies of each bed,qualified rate of hand microorganisms,and infection sites were observed before the intervention(from Oct 2011 to May 2013)and after the intervention(from Jun 2013 to Jan 2015).The statistical analysis of data was performed with the use of SPSS16.0software.RESULTS The infection rate of the hospitalized patients of the oncology department was 4.67% after the intervention,significantly lower than 9.67% before the intervention(P〈0.01).Among the long-term hospitalized patients with infections,the incidence rates of upper respiratory tract infections,lower respiratory tract infections,gastrointestinal tract infections,and urinary tract infections were respectively 3.67%,3.33%,3.67%,and5.33% before the intervention and were respectively 2.33%,2.33%,2.67%,and 2.33% after the intervention,and there was significant difference(P〈0.05).There was significant difference in the implementation rate of hand hygiene,qualified rate of hand disinfection supplies,or qualified rate of hand microorganisms before and after the intervention(P〈0.05).CONCLUSIONThe comprehensive intervention to hand hygiene can raise the implementation rate of hand hygiene of the health care workers,improve the qualified rate of hand microorganisms,and effectively reduce the incidence of nosocomial infections in the patients of oncology department.
