摘  要:目的了解全科医学专业学位硕士研究生教育与住院医师规范化培训"并轨"的效果及存在的问题,并提出对策。方法以首都医科大学全科医学与继续教育学院第七临床学系2012—2014级的13名规范化培训研究生为访谈对象。采用小组访谈方法进行调查,访谈内容包括临床科室轮转安排是否合理、对全科医学的认知度与学科/科室归属感、带教师资关于全科理念或全科思维的教学情况、生活待遇满意程度、对"并轨"培养方式的认知及对培训质量影响因素的认知等。结果所有学员均表示目前的科室轮转安排大致合理。全科医学的深入学习使得所有学员表示对该专业有一定了解,并认为全科医学具有良好前景。对于临床基地师资带教中是否融入全科思维,有10人认为很少。在生活待遇方面,学员们表示无需父母资助即可满足日常花销。"并轨"后"5+3"模式与"并轨"前"5+3+2/3"模式相比,所有学员均认为前者更合理。影响研究生培训质量的因素排在前三位的依次为:临床轮转(8/13)、学科水平(3/13)、导师指导(2/13)。结论全科医学专业学位硕士研究生教育与住院医师规范化培训"并轨"是教育医疗改革的探索,可以提高全科医学人才产出效率,但需要提高带教师资能力,特别是将全科医学理念贯穿带教过程。加强对现有全科临床、社区基地师资人员的培训,完善社区实习基地建设,才能促进全科医学卫生事业的发展。Objective To understand the difficulties and problems of the integration mode of the postgraduate education for the professional degree of general practice and standardized training for residency of general practitioners and put forward countermeasures. Methods We selected 13 postgraduate students of 2012—2014 grade of the Seventh Department of Clinical Medicine of the School of General Practice & Continued Education of Capital Medical University who received standardized training. Group interview was undertaken,and the interview content concluded whether the rotation arrangement of clinical departments was rational,the cognition degree toward general practice and the sense of belonging to subject/department, the teaching of the concepts and thinking of general practice,the satisfaction degree with material amenities,the cognition toward the integrated training mode and the cognition toward the influencing factors for training quality. Results All respondents said that the current rotation arrangement was rational. All the respondents said the further study of general practice made them acquire a certain amount of knowledge about the subject and made them believe that the subject has a well potential. There were 10 respondents thought only little concept of general practice was merged into the teaching at clinical bases. In the aspect of material amenities,the students said that the daily expense could be covered without the financial support of their parents. All students thought the“5﹢3”mode after integration was superior to the“5﹢3﹢2/3”mode before integration. The influencing factors for quality of the postgraduate training were clinical rotation(8/13),subject level(3/13)and tutors' guidance(2/13). Conclusion As an exploration of educational and medical reform,the integrated mode can increase the efficiency of cultivating general practitioners,while the competency of teaching staff should be improved and more general practice concepts should be merged into the teaching.
