摘 要:目的:探讨超声弹性成像(ultrasound elastography,UE)与彩色多普勒血流成像(color Doppler flow imaging,CDFI)在诊断乳腺良恶性病变中的价值。方法:对经病理证实的138例患者143个乳腺肿块进行CDFI与UE检查的回顾性分析。病灶的血流分级采用Adler半定量法;UE评分采用5分法。结果:143个病灶中,良性病灶107个、恶性病灶36个。CDFI诊断乳腺恶性肿块的敏感性为83.3%,特异性为91.6%,准确性为89.5%;UE诊断乳腺恶性肿块的敏感性为91.6%,特异性为95.3%,准确性为94.4%。结论:UE在诊断敏感性、特异性和准确性方面较CDFI有明显的优势,临床鉴别乳腺实性病灶良恶性时,CDFI联合UE有助于提高乳腺恶性肿瘤诊断的准确性。ObjectiveTo discuss the values of ultrasound elastography (UE) and color Doppler uhrasonography (CDFI) for diagnosing benign and malignant breast lesions. Methods TotalLy 143 breast lumps from 138 patients confirmed pathoh)gically underwent retrospective analysis of CDFI and UE examinations. The TIMI flow grades of lesions were analyzed by Adler semi-quantitative method, and the UE lesions were analyzed by 5 points method. Results In the 143 breast lumps, there were 107 benign lesions and 36 malignant lesions. The sensitivity, specificity and accuracy of CDFI for diagnosing breast malignant lesion were 83.3%, 91.6% and 89.5% respectively, while the ones of UE were 91.6%, 95.3% and 94.4% respectively. Conchuslon UE behaves better than CDFI in sensitivity, specificity and accuracy. The accuracy for diagnosing the breast malignant tumor diagnosis could be improved by combined CDFI and UE.
