南宁市2000例学龄前儿童血铅水平与5种必需元素关系分析

摘　　要：目的研究南宁市2 000例0~6岁儿童（男1 298名,女702名）血Pb含量及与Zn、Cu、Fe、Ca、Mg含量的关系。方法用原子吸收分光光度法测定血Pb、Zn、Cu、Fe、Ca、Mg含量,为便于与国内外文献比较,以血Pb浓度≥100μg/L为铅中毒判定标准。结果学龄前儿童平均血铅水平（BLL）为（52.11±45.10）μg/L,铅中毒检出率为6.60%。以4~岁儿童BLL、铅中毒检出率最高。男童BLL高于女童,血Fe含量低于女童,差异有统计学意义（P〈0.05）。BLL 50~μg/L组与BLL〈50μg/L组比较,血Zn、Fe含量较高,Ca含量较低,差异有统计学意义（P〈0.05）。相关性分析显示,BLL〈50μg/L组BLL与血Zn、Cu、Fe含量及年龄呈正相关,与Ca含量呈负相关;BLL〉150μg/L组BLL与血Mg含量呈正相关。结论儿童BLL和血Fe水平具有性别差异。低水平Pb暴露可引起体内Zn、Cu、Fe、Ca、Mg元素代谢及平衡紊乱。Objective Our study aimed to explore the distribution of blood lead level（BLL）and its relationship to 5essential elements in 2000 preschool children from Nanning,Guangxi Province,China.Methods Design and methods Levels of lead,zinc,copper,iron,calcium,and magnesium in whole blood samples of 2000 children aged 0-6-year-old were determined using atomic absorption spectrometry.Intoxication level was defined as BLL≥100μg/L.Results The mean of BLL was 52.11±45.10μg/L with an intoxication prevalence of 6.60%.The highest BLL and intoxication prevalence were found in the group of children aged among 4to 5years old.Gender-differences were found in blood lead and iron levels.Compared to the group of children with BLLs〈50μg/L,children with higher BLLs obviously had higher blood zinc and iron levels,and lower blood calcium levels.In the group of children with BLLs〈50μg/L,BLL showed a positive correlation with age and blood zinc,copper,iron and magnesium levels,and a negative correlation with blood calcium level.Conclusions Metabolic and balance disorder of essential elements were found at a low BLL which currently considered safe.
