慢阻肺患者血清白介素-17、白介素-23及和肽素的变化及意义

摘　　要：目的探讨慢性阻塞性肺疾病(简称慢阻肺)患者血清白介素-17(IL-17)、白介素-23(IL-23)及和肽素(Copeptin)的变化及意义。方法选取2014年2月-2015年2月我院收治的慢阻肺急性加重期(AECOPD)患者40例作为观察组1,慢阻肺稳定期患者40例作为观察组2,选取同期体检健康者40例分别作为对照组。检测并对比三组患者的血清IL-17、IL-23、Copeptin及CRP水平。结果观察组1血清L-17、IL-23、Copeptin及CRP水平明显高于观察组2和对照组,观察组2明显高于对照组,差异具有统计学意义(P<0.05)。观察组1合并呼吸衰竭患者的血清L-17、IL-23、Copeptin及CRP水平均明显高于未合并呼吸衰竭患者,差异具有统计学意义(P<0.05)。经Pearson相关性分析:血清IL-17、IL-23、Copeptin水平均分别与血清CRP水平呈直线正相关(r=0.682、0.664、0.705,P<0.05);血清IL-17、IL-23水平之间呈直线正相关(r=0.653,P<0.05),血清IL-17、IL-23分别与血清Copeptin水平呈直线正相关(r=0.694、0.705,P<0.05)。结论血清IL-17、IL-23及Copeptin可反映慢阻肺患者炎性反应程度,与疾病的病情存在一定的关系。Objective To investigate the changes and significance of the serum interleukin – 17（ IL-17）,interleukin-23（ IL-23） and Copeptin in chronic obstructive pulmonary disease（ COPD） patients. Methods 40 AECOPD patients were selected as the observation group 1,40 COPD patients at stable stage were taken as the observation group 2,and 40 physical healthy subjects as the control group. The levels of serum IL-17,IL-23,Copeptin and CRP were recorded and compared among the three groups. Results The levels of serum L-17,IL-23,Copeptin and CRP were the highest in the observation group 1,followed by the observation group 2 and the control group（ P〈0. 05）. The levels of serum L-17,IL-23,Copeptin and CRP were significantly higher in patients complicated with respiratory failure than those without respiratory failure in the observation group 1（ P〈0. 05）. The Pearson correlation analysis showed the levels of serum IL-17,IL-23 and Copeptin had the positively linear correlation with the serum CRP level（ r = 0. 682,0. 664,0. 705,P〈0. 05）. The serum level of IL-17 had the positively linear correlation with the serum IL-23 level（ r = 0. 653,P〈0. 05）. The levels of serum IL-17 and IL-23 had the positively linear correlation with the level of serum Copeptin（ r = 0. 694,0. 705,P〈0. 05）. Conclusion The levels of serum IL-17,IL-23 and Copeptin can reflect the degree of inflammatory reaction in patients with COPD,and they have certain relationship with the state of illness.
