快速康复理念在肺癌全肺切除术患者围手术期护理中的应用

摘　　要：目的:探讨快速康复理念(FTS)在肺癌全肺切除术患者围手术期护理中的应用效果。方法:将92例肺癌全肺切除手术患者随机等分为FTS组和对照组。FTS组患者接受FTS治疗及护理,对照组患者围手术期接受传统方式护理。比较两组患者的疗效、并发症及疼痛情况。结果:术后FTS组白细胞水平低于对照组(P<0.05),术后白蛋白、总蛋白水平高于对照组(P<0.05),FTS组手术时间、术中出血量和对照组比较,差异无统计学意义(P>0.05)。FTS组术后并发症发生率少于对照组(P<0.05),术后住院时间短于对照组(P<0.05),总医疗费用少于对照组(P<0.05),患者体重下降情况少于对照组(P<0.05),术后各个时间点的VAS评分低于对照组(P<0.05)。结论:FTS应用于肺癌手术,可以减轻患者术后疼痛程度,减轻炎症反应,减少营养流失,缩短住院时间,降低医疗成本,提高综合疗效,且安全性高,有利于患者尽快康复。Objective ： To explore the effect of applying the fast - track surgery （FI＇S） in perioperative nursing of patients with lung cancer and who had gone through pneumonectomy. Methods：Ninety - two cases of lung cancer patients and who had gone through pneumonectomy were randomly divided into the FFS group and the eontrol group. Patients in the FTS group received FTS treatment and nursing, and the control group received the traditional nursing care in perioperative period. We compared the efficacies, complications and pains of the two groups. Results ：The post - operation leucocyte level in the FTS group was lower than that of the control group （ P 〈 0.05 ）, and the post - operation total protein levels were higher than those in the control group （P 〈 0.05 ）. The differences of operation times, intra - operative amounts of bleeding between the two groups had no statistical significance （ P 〉 0.05 ）. The incidence rate of post - operative complications in the FI＇S group was lower than that of the control group （ P 〈 0.05 ）. The post - operative hospitalization times in the FTS group were shorter than those of the control group （ P 〈 0, 05 ）. The total medical costs in the FIB group were less than those in the control group （ P 〈 0.05）. The losses of weights of patients in the FTS group were less than those of the control group （P 〈 O. 05 ）. And the VAS score in each time was lower than that of the control group （P 〈0.05）. Conclusion：The application of FTS in lung cancer surgery can reduee post - operative pain,inflammatory reac- tion,and the loss of nutrition,and shorten the length of hospital stay,reduce medical costs,improve the＇ comprehensive efficacy. Besides,the FTS is of high safety and would help patients reeover as soon as possible.
