细胞因子IL-15、IL-16、IFN-γ及TGF-β在慢性乙型肝炎病毒感染者外周血中的表达及临床意义的研究

摘　　要：目的探讨HBV感染者外周血细胞因子IL-15、IL-16、IFN-γ、TGF-β的表达水平变化的及临床意义。方法选择肝癌患者60例(HCC组)、肝硬化患者60例(LC组)、慢性乙型肝炎患者60例(CHB组)、慢性乙型肝炎病毒携带者60例(ASC组)和健康对照者40例(健康对照组),采用酶联免疫法检测细胞因子表达水平,运用聚合酶链反应检测HBV DNA载量,同时检测各项生化指标。结果随着患者病情加重,细胞因子IL-15、IL-16、IFN-γ、TGF-β表达水平均呈逐渐升高的趋势,且在肝硬化患者中达到最大值。与健康对照组相比,不同临床类型HBV感染者细胞因子表达水平均显著升高,差异有统计学意义(P<0.05)。IL-15、IL-16、IFN-γ、TGF-β在CHB患者外周血HBV DNA高载量组表达水平最高,差异有统计学意义(P<0.05)。结论 IL-15、IL-16、IFN-γ、TGF-β可能参与了乙型肝炎的肝脏炎性损伤过程,且其表达水平与病毒量具有一定相关性。Objective To investigate the expression levels of peripheral blood cytokines（IL-15,IL-16,IFN-γand TGF-β）in the patients with hepatitis B virus（HBV group）infections and its clinical significance.Methods 60 cases of hepatocellular cancer（HCC）,60 cases of liver cirrhosis（LC group）,60 cases of chronic hepatitis B（CHB group）,60 hepatitis B virus carriers（ASC group and 40 healthy control individuals（control group）were selected.The expression levels of cytokines were detected by using the enzyme linked immunosorbent assay（ELISA）and HBV DNA loads were measured by using real time fluorescence quantitative polymerase chain reaction.Meanwhile,the biochemical indexes were detected.Results With the disease aggravation,the expression levels of cytokines IL-15,IL-16,IFN-γand TGF-βshowed a gradually increasing trend,and reached a maximum value in the patients with liver cirrhosis.In the comparison with the healthy control group,the expression level of cytokines were significant increased in different clinical types of HBV infections,the differences were statistically significant（P〈0.05 or P〈0.01）.The expression levels of IL-15,IL-16,IFN-γand TGF-βwere highest in the high HBV DNA loads group among the patients with CHB,the difference was statistically significant（P〈0.05）.Conclusion IL-15,IL-16,IFN-γand TGF-βmight be involved in the liver inflammatory injury process in hepatitis B,moreover their expression levels have certain correlation with the viral quantity.
