参斛汤联合吡柔比星膀胱灌注对膀胱癌术后患者不良反应、生存质量的影响

摘　　要：目的：观察参斛汤联合吡柔比星膀胱灌注对膀胱癌术后患者不良反应、生存质量的影响。方法：将2011年4月—2013年2月本院泌尿外科收治的112例膀胱癌术后患者按照随机数字表法分为对照组和观察组患者各56例。对照组给予术后吡柔比星膀胱灌注,每周1次,8次后每月1次;观察组则在膀胱灌注过程中辅助加用参斛汤内服,两组均连续治疗1 a。观察两组患者临床疗效及相关指标变化情况。结果：观察组患者不良反应发生率显著低于对照组,差异有统计学意义（χ^2=5.009,P=0.043〈0.05）;术后灌注治疗6个月、1 a后,观察组患者不同时间段的生存质量相关的躯体、角色、认知、感情、社会等5个功能领域评分显著高于对照组,差异具有统计学意义（P〈0.05）;观察组术后2 a复发率显著低于对照组,差异性显著（P〈0.05）。结论：参斛汤辅助应用于吡柔比星膀胱灌注治疗过程中,利于强化临床抗肿瘤疗效,减少灌注过程中的不良反应。Objective： To observe the influence of Shen Hu Decoction（ 参斛汤,SHD） in combination with pirarubicin on adverse reactions and life quality of patients after receiving the operation on bladder cancer. Methods： One hundred and twelve patients who received the operation on bladder cancer in our hospital＇s urinary surgery department between April of 2011 and Feb. of 2013 were divided into the control group and the observation group according to random number table method,with 56 cases in each group. The ones in the control group were given pirarubicin for bladder perfusion,once each week at first,then once each months after treating 8 times,while the ones in the observation group were added SHD based on the treatment of the control group. The two groups were both treated for one year continuously,then their clinical curative effects and the changes of related indexes were observed. Results： The occurrence rate of adverse reactions of the observation group was much lower than that of the control group,and the difference between them had statistical significance（ χ^2= 5. 009,P = 0. 043 0. 05）. Six months later and one year later after postoperative infusion respectively,the five functional areas＇ scores including body,role,recognition,feeling and society which are related to life quality in different stages were much higher than those of the control group,and the differences between them had statistical significance（ P〈0. 05）. The reoccurrence rate of the observation group 2 years later after the operation was much lower than that of the control group,and the difference between them had significance（ P〈0. 05）. Conclusion： SHD in combination with pirarubicin for bladder perfusion can improve the clinical curative effect against tumors,and reduce adverse reactions.
