2014年南宁市梅毒、淋病报告病例的准确性分析

摘　　要：目的了解南宁市梅毒与淋病网络报告病例的准确性,为制定防治政策提供参考依据。方法抽取南宁市各级医疗机构13家,每家随机抽取梅毒、淋病病例各20例,根据国家及广西壮族自治区的相关标准进行病例核实。结果 2014年南宁市梅毒、淋病报告病例的准确性分别为47.55%和84.62%,其中一期梅毒62.96%,二期梅毒37.84%,三期梅毒62.50%,隐性梅毒48.39%,胎传梅毒仅为12.50%。不同等级医疗机构淋病报告病例的准确性差异有统计学意义(P<0.001),梅毒报告准确性差异无统计学意义(P=0.088)。有无皮肤性病科对医疗机构的梅毒、淋病报告准确性均无统计学影响。设有规范化皮肤性病门诊的医疗机构的梅毒报告病例准确性(66.00%)高于未开设的医疗机构(29.81%),差异有统计学意义(P<0.001);而淋病方面差异则无统计学意义(P=0.741)。结论南宁市梅毒报告病例的准确率仍较低,需进一步加强培训和督导,提高南宁市梅毒、淋病报告病例的准确性。Objective To study the diagnostic accuracy of notifiable syphilis ＆ gonorrhea cases registered in network in Nanning prefecture 2014, so as to provide scientific evidence for decision-making of syphilis ＆ gonorrhea. Methods Thirteen hospitals at different level were selected. Twenty notifiable syphilis cases and 20 gonorrhea cases were sampled randomly in each hospital. The diagnostis of notifiable syphilis ＆ gonorrhea cases was checked according to the professional hygiene standards of syphilis ＆ gonorrhea. Results The diagnostic accuracy rates for syphilis and gonorrhea were 47.55% and 84. 62% , respectively. Accuracy rates for the primary syphilis, the secondary syphilis, the tertiary syphilis, the latent syphilis and the congenital syph- ilis were 62. 96% , 37.84%, 62. 50% , 48.39% and 12. 50% , respectively. The diagnostic accuracy rate of Gonorrhoea was statistically different among hospitals at distinct level Compared with hospitals without standard outpatient service for Dermatology ＆ Venereology, the accuracy rate of syphilis reported by hospitals with such service was statistically higher. Conclusion The diagnostic accuracy rates of syphilis were still quite poor in Nanning. Measures on training staff and supervising the reporting system are urgently taken to improve the accuracy of syphilis.
