丙烯酸所致职业性痤疮1例

摘　　要：患者男,63岁。全身起疹伴痒3月余。起疹前接触丙烯酸。皮肤科情况:面部弥漫性淡青色斑,其上密集黑头粉刺,伴明显的毛囊口角化,散在囊肿、结节,间有少许粟粒样皮疹。躯干、四肢见较多黑头粉刺,少许炎性丘疹,散在草黄色囊肿。皮损组织病理示:表皮大致正常,真皮内可见一囊腔,囊壁为复层鳞状上皮,囊腔内容物为板层状的角质物,少许角化不全细胞,视野左侧可见真皮浅层成纤维细胞及胶原增生,伴血管增生。诊断:职业性痤疮。A 63-years-old male presented with rash accompanied by itching on the whole body fi）r more thau 3 months. The patient had a acrylic contacting history before rash. The dermatological examination showed faeial diffuse ,oale blue spots with intensive I）laekheads, accornpanied by significant keratinization of hair follieles, scatlered cysts and nodules, between which there is little rniliary rash. Many blackheads a few inflammatory papales. and scattered yellow trysts were observed on the Torso and limbs. The histopathologie examination showed that normal epidermis, a intradermal cyst consisting of stratified squamous epithelium on cyst wall and the. lamellar horny substaHce and a little parakemtosis cells in the cyst. There were. Hyperproliferation of fibroblasts, collagen and wtseular was observed in the superficial layer of dermis in tire left visual fiehl. A diaguosis of occupational ache was made.
