润燥止痒胶囊联合左西替利嗪治疗对慢性自发性荨麻疹患者的影响

摘　　要：目的观察润燥止痒胶囊联合左西替利嗪治疗慢性自发性荨麻疹患者的临床疗效及其对患者血清IgE及类胰蛋白酶水平的影响。方法将入选的100例慢性自发性荨麻疹患者随机分为两组:联合治疗组与单一治疗组。联合治疗组予左西替利嗪和润燥止痒胶囊口服,单一治疗组仅予左西替利嗪口服。治疗前后采用化学发光免疫法检测患者血清总IgE水平及类胰蛋白酶水平,同时检测30名健康志愿者作为阴性对照。结果联合治疗组有效率(93.75%)明显高于单一治疗组(62.00%)(P<0.01);两组患者治疗前血清中总IgE及类胰蛋白酶水平均高于健康志愿者组,两组患者治疗前、后血清总IgE与类胰蛋白酶水平均呈正相关(P均<0.05)。联合治疗后血清总IgE水平和血清类胰蛋白酶水平均下降(P<0.05),而单一治疗组仅血清类胰蛋白酶水平较治疗前降低(P<0.05)。结论润燥止痒胶囊联合左西替利嗪对慢性自发性荨麻疹的临床疗效比单用左西替利嗪好,且能有效降低患者血清总IgE及类胰蛋白酶水平。Objective To observe the clinical efficacy of Run-Zao Zhi-Yang capsule combined with levocetirizine in the treatment of chronic spontaneous urticaria and its influence on the serum level of IgE and tryptase. Methods One hundred patients with chronic spontaneous urticaria were randomly divided into two groups, the combination treatment group treated by both levocetirizine and Run-Zao Zhi-Yang capsule, and the single treatment group only treated by levocetirizine. The serum level of IgE and tryptase of the patients were detected with chemiluminescence immunoassay before and after treatment. As a negative control, thirty healthy volunteers were detected. Results The effective rate of the combination treatment group （93.75%） was significantly higher than that of the single treatment group （62.00%） （ P 〈 0. 01）. Before the treatment, the serum level of either IgE or tryptase in both patient-groups was significantly higher than healthy volun- teers respectively. The serum level of IgE was positively correlated （ P 〈 0. 05 ） with tryptase in two groups before and after treatment. The serum level of total IgE and tryptase in the combination treatment group was decreased after the treatment （ P 〈 0. 05 ）. In the single treatment group, only the serum level of tryptase was significantly reduced （ P 〈 0. 05 ）. Conclusion The clinical effect of the Run-Zao Zhi-Yang capsules combined with levocetirizine therapy is better than that of the levocetirizine single therapy in chronic spontaneous urticaria, which can significantly reduce the serum level of both IgE and tryptase in those patients.
