鼻内镜术后白眉蛇毒血凝酶致低纤维蛋白原血症1例

摘　　要：1病例报告患者,男,61岁,因反复鼻塞流脓涕20余年于2015年9月7日入院。既往有糖尿病史,口服二甲双胍（1g/次,1次/d）,血糖控制可。未曾服用任何抗凝血药物。鼻内镜检查见双侧钩突明显肥大、息肉样变,双侧中鼻道可见黄白色米汤样分泌物。鼻窦CT示双则上颌窦充满等密度软组织影,其内混杂斑片状高密度影。门诊以＂真菌性鼻窦炎（双）、鼻息肉（双）、糖尿病＂收入院。术前检查血常规、A 61year-old male patient,plasma fibrinogen level was 2.98g/L,endoscopic sinus surgery was performed under general anesthesia for polypoid of uncinate process with mycotic maxillary sinusitis.Hemocoagulase were given in pre-and post-operative for treatment.The patient was found postoperative drain blood continuously since 3days after surgery,when the dose of hemocoagulase reach 26 KU,and fibrinogen determined in Plasma was 0.48g/L.Coagulation returned to normal and nasal bleeding stopped after discontinuation of the hemocoagulase and supplement with fibrinogen.
