AIDS患者HAART治疗免疫重建过程中检测CD4^+CD45RO^+/CD45RA^+CD127^+T细胞的意义及其相关免疫调节分析

摘　　要：目的探讨CD4^+CD45RO^+/CD45RA^+CD127^+T细胞在AIDS病程中的评价及其临床意义。方法采用流式细胞术检测经高效抗逆转录病毒治疗(HAART)的AIDS的CD4^+以及CD8^+T细胞表面CD127分子的表达,以CD4^+T细胞绝对数分为三组:A组,<350个/mm^3;B组,350~800个/mm^3;C组,>800个/mm^3。同时用荧光定量PCR技术检测AIDS患者血浆中HIV(病毒载量小于50 copies/ml,病情稳定者),用流式细胞术检测上述患者外周血B淋巴细胞及自然杀伤细胞表面分子CD19、CD8/38和CD56的表达,比较组间差异。结果在经HAART有效的39例AIDS中,其CD4^+CD45 RO^+CD127^+T细胞数与患者CD4^+、CD4^+CD45 RA^+和CD4^+CD45RO^+细胞数呈显著正相关(相关系数分别为0.649、0.539和0.977,P<0.05);CD4^+CD45RA^+CD127^+与患者CD4^+、CD4^+CD45RO^+和CD4^+CD45RA^+有显著相关性(相关系数分别为0.550、0.497和0.992,p<0.05)。CD4^+CD45RA^+、CD4^+CD45RO^+、CD4^+CD45RA^+CD127^+和CD4^+CD45RO^+CD127^+标记CD19分子组间比较均有统计学差异(P<0.05)。结论在AIDS免疫重建过程中,CD4^+T细胞亚群在对抗病毒的复制起了主要作用,CD4^+CD45RO^+CD127^+T调节细胞群对于AIDS患者的免疫重建作用比较重要。Objective To investigate the evaluation and clinical significance of CD4＋ CD45RO＋ / CIN5RA＋CD127＋ T cells in the course of AIDS patients. Methods The expressions of virgin and memory CD4＋ and CD8＋ T cell surface CD127 molecules were detected by flow cytometry in AIDS patients undergoing highly active anti-retroviral therapy （HAART）, who were divided into three groups of A （with the absolute number of CD4＋ T cells less than 350 pieces/mm3 ）, B （350-800 pieces/mm3） and C（more than 800 pieces/mm3 ）. The expressions of CD19,CD8/38 and CD56 on the surface molecules of B lymphocytes and natural killer cells were detected by flow cytometry. Meanwhile, HIV virus in plasma was detected by fluorescent quantitation PCR in AIDS patients with stable status and viral load less than 50 copies/ml. The results were compared among four groups. Results In 39 patients with effective HAART, the CD4＋ CIMSRO＋ CD127＋ T cell count was positively correlated with CIN＋ , CD4＋ CD45RA＋ and CD4＋ CD45RO＋ CD127＋ （r=0. 649, 0. 539 and0.997, P〈0.05）, so did the CD4＋CD45RA＋CD127＋ and CD4＋, CD4＋CD45RO＋ and CD4＋ CD45RA＋ （r=0. 550,0. 497 and 0. 992,P〈0.05）. The significant differences existed in CD4＋ CD45RA＋ , CD4＋ CIN5RO＋ , CD4 ＋ CD45RA＋ CD127＋ and CD4＋ CIN5RO＋ CD127＋ marked molicule CD19 among four groups（P〈0. 05）. Conclusion The CD4＋T cell groups take a very important role in against virus Copying in the process of HIV immune reconstitution, in which the CD4＋ CD45RO＋ CDI27＋ T cell play a more important role in the patients with AIDS.
