摘 要:目的调查胃肠外科住院患者营养风险发生率、营养支持应用状况及营养支持过程中不良反应发生情况。方法对208例胃肠外科住院患者使用营养风险筛查2002(NRS2002),并调查患者住院期间的营养支持状况,分析营养风险与营养支持的关系,采用NRS2002工具,根据患者体重指数(BMI)、近期体质量变化、饮食摄入变化和原发病对营养状况影响的严重程度等四方面情况进行计算评分。并对患者营养支持情况进行调查。结果胃肠外科NRS2002总适用率为79.7%,营养风险发生率为62.7%,总营养支持率85.3%;64例有营养风险患者中59例(92.2%)接受了营养支持;无营养风险的39例患者中28例(71.8%)接受了营养支持;在使用"全合一"的患者中仅40.9%符合热氮比要求,仅有38.6%糖脂热量比合理;不良反应发生率32.4%。结论 NRS2002适用于胃肠外科住院患者的营养筛查,但营养支持在处方组成、使用疗程等方面存在不合理现象。建议胃肠外科对有营养风险的患者规范合理地给予营养支持,以有效地改善预后,减少不良反应发生率。Objective To investigate the incidence of malnutrition risk,nutritional status and adverse reactions during the nutritional support for gastrointestinal surgical inpatients. Methods A total of 208 cases of gastrointestinal surgical inpatients were selected for investigation,and the Nutritional Risk Screening 2002( NRS2002) was used to investigate the relationship between nutritional support and nutritional risk for inpatients.The patient's body mass index( BMI),weight change,changes in dietary intake and impact of primary disease were recorded. Results The overall applicable rate of NRS2002 for gastrointestinal surgical inpatients was 79.7%,the incidence of nutritional risk was 62. 7%,and the total nutritional support rate was 85.3%.Fifty-nine cases among 64 with nutritional risk( 92.2%) received nutritional support,while 28 cases among 39 without nutritional risk( 71.8%) received nutritional support.Among patients receiving the " all in one" parenteral nutrition,only 40.9%met the requirement for calorie-to-nitrogen ratio,and only 38. 6% glycolipid calories ratio was reasonable,incidence of adverse reactions in patient receiving nutritional support was 32.4%. Conclusion NRS 2002 tool is suitable for nutritional screening in gastrointestinal surgical inpatients,but some inappropriate drug use exists during nutritional support,such as nutritional composition and course of treatment,etc. It is suggested that gastrointestinal surgical inpatients should be rationally administered with nutritional support,in order to improve prognosis and reduce the incidence of adverse reactions.
