机械力量脊椎矫正治疗老年性腰痛的临床疗效研究

摘　　要：目的本研究机械力量脊椎矫正治疗老年性腰痛患者的疼痛和功能障碍的临床疗效。方法采用随机对照前瞻性临床研究方法，人选60例年龄在60岁以上、病程在6W以上、伴有或不伴有下肢放射痛的机械性腰痛患者，随机分配到研究组（机械力量脊椎矫正）和常规组（常规物理因子治疗）。主观指标包括疼痛（VAS）和功能障碍（RDQ），在两组治疗前、治疗4W后评估以观察临床疗效。结果两组腰痛患者在治疗后的疼痛和功能障碍与治疗前相比都显著下降（P〈0．05），然而接受机械力量脊椎矫正的患者与接受常规物理因子治疗的患者相比，疼痛值降低更显著（P〈0．05）。结论机械力量脊椎矫正治疗老年性腰痛疗效满意，较常规物理因子治疗效果更好。Objective To assess the efficacy of spinal mechanical manipulation and conventional physiotherapy on el- derly patients with LBP by subjective pain and dysfunction. Methods A randomized clinical trial and follow-up study design was used. A total of 60 subjects met the inclusion/exclusion criteria, at least 60 years elderly, who had a primary complaint of LBP of at least 6-week duration with or without radiating pain to the lower extremity were included in this trial, and ran- domized to either spinal mechanical manipulation or conventional physiotherapy. A 100mm visual analogue scale （VAS） for perceived pain, the Roland-Morris dysfunction questionnaire （RDQ） for low back function served as primary outcome measures. This prospective randomized clinical trial examined the relative efficacy of spinal mechanical manipulation and conventional physiotherapy for the treatment of mechanical LBP of at least 6-week duration. Results Pain and disability had a decrease after intervention regardless of which group the attended （P 〈 0.05）, however, subjects who received spinal mech- anical manipulation had significantly lower pain scores than subjects who received conventional physiotherapy （P〈 0.05）. Conclusion Spinal mechanical manipulation was found to be more effective in reducing pain than conventional physiotherapy when treating older patients with LBP.
